| Upon arrival on 3/3/2016 the well pressure is at 100 psi. Nicks began to relieve the pressure from the well. It took less than one minute for the well to blow down. The wellbore was then loaded with brine to prepare for the well to be logged. Appalachian Well Survey is on location to preform the logging operation. Appalachian sent their tools down the 2 3/8 tubing and established total depth of the well at 834. The 2 3/8 tubing is set to a depth of 232 and is attached to a formation packer at the base of the tubing. The well is open hole from the base of the tubing to total depth. Once the logging operation was complete and Appalachian Well Survey was off location, the well was topped off with brine and shut in until 3/4/2016.

The rig crew removed their tools from the well and loaded the wellbore with 10 BBLs of brine water. The well went on vacuum and then became static. The wellhead was shut in until 3/3/2016. The well is scheduled to be logged on 3/3/2016.

Nicks ran new 2 3/8 diameter tubing into the well to 190. The well was packed off and the cementers rigged up to the 2 3/8 tubing. Nicks broke circulation and conditioned the wellbore. Once the wellbore was conditioned, Nicks pumped 45 sacks of Class A cement that scaled at 15.6 ppg. The cement slurry was pumped until good cement returns were observed at surface. The tubing was removed from the well and the well was shut in until 3/9/2016.

The crew has placed orange safety fence around the location. The well was shut in and the crew will return on 3/2/2016 to assemble a wellhead on the well.

Todays inspection found a new room being built on the north side of the dwelling, in the area of the plugged well. This construction prohibited taking a GPS reading. Due to this construction, there was no restoration required after plugging the well as all areas affected by the well plugging are either within the construction zone or meet restoration requirements. Final Restoration is passed. Pictures were taken today and are located in the Orphan well file for this well.

The well was packed in and plumbed to the steel pit that was on location. Nicks cementers rigged up to the wellhead and began to circulate 12 BBLs of fresh water to condition the wellbore. Once the wellbore was condition, they began to pump 42 sacks of Class A cement that scaled at 14.8 ppg. The cement slurry was pumped at a rate of 1 BBL per minute at 500 psi. The slurry was displaced with .57 BBLs of fresh water. Nicks removed 12 joints of 1 tubing from the well. The cementers rigged back up to the tubing, broke circulation and pumped 42 sacks of Class A cement that scaled at 14.8 ppg. The slurry was displaced with .24 BBLs of fresh water. Nicks then removed the tubing from the well and shut the well in until 3/7/2016.
