0214930586 2016-03-03 Plug / Plug Back No Violations Upon arrival on 3/3/2016 the well pressure is at 100 psi. Nicks began to relieve the pressure from the well. It took less than one minute for the well to blow down. The wellbore was then loaded with brine to prepare for the well to be logged. Appalachian Well Survey is on location to preform the logging operation. Appalachian sent their tools down the 2 3/8 tubing and established total depth of the well at 834. The 2 3/8 tubing is set to a depth of 232 and is attached to a formation packer at the base of the tubing. The well is open hole from the base of the tubing to total depth. Once the logging operation was complete and Appalachian Well Survey was off location, the well was topped off with brine and shut in until 3/4/2016.
1354889117 2016-03-02 Plug / Plug Back No Violations Upon arrival on 3/2/2016 the pressure on the well is 120 psi. Nicks began to relieve the pressure from the well. Once the pressure on the well was blown down enough, the crew removed the domestic feed from the well and installed a wellhead. The rig crew went in and tagged the total depth of the well at 855. The rig crew removed their tools from the well and loaded the wellbore with 10 BBLs of brine water. The well went on vacuum and then became static. The wellhead was shut in until 3/3/2016. The well is scheduled to be logged on 3/3/2016.
1360985507 2016-03-08 Plug / Plug Back No Violations On 3/8/2016 the pressure on the well was 0 psi. Nicks ran tools into the 2 3/8 tubing to determine if the cement held at 225. The cement tagged at 235. Appalachian Well Survey is on location to shoot the 2 3/8 tubing off at 205. Once the tubing was shot off, Nicks removed the old 2 3/8 diameter tubing from the wellbore. Nicks ran new 2 3/8 diameter tubing into the well to 190. The well was packed off and the cementers rigged up to the 2 3/8 tubing. Nicks broke circulation and conditioned the wellbore. Once the wellbore was conditioned, Nicks pumped 45 sacks of Class A cement that scaled at 15.6 ppg. The cement slurry was pumped until good cement returns were observed at surface. The tubing was removed from the well and the well was shut in until 3/9/2016.
1457640332 2016-03-01 Plug / Plug Back No Violations I was on location to cover the operations for this orphan well plug job. Nicks Well Plugging will be the plugging contractor. The well is constructed with 6 casing and 2 3/8 tubing. There is no wellhead in place on the well. Pressure on the well at the time of arrival is 175 psi. They are in the process of blowing the well down and moving equipment on to the location. The crew has placed orange safety fence around the location. The well was shut in and the crew will return on 3/2/2016 to assemble a wellhead on the well.
1457703786 2016-03-07 Plug / Plug Back No Violations On 3/7/2016 the wellhead pressure is at 10 psi. Nicks relieved the pressure from the well and it blew down quickly. Nicks crew ran tools down the 2 3/8 tubing to tag the top of the cement. Cement was tagged at 307 and needs to be at 232. Nicks ran 1 tubing into the 2 3/8 tubing to 300. The well was packed off and the cementers rigged up to the 1 tubing. The cementers broke circulation and pumped 20 sacks of Class A cement that scaled at 15.6 ppg. The slurry was pumped at a rate of 1 BBL per minute at 1000 psi. The tubing was removed from the well and the well was shut in until 3/8/2016.
1464706093 2016-05-18 Final Restoration No Violations Mr. Feher, the landowner, plugged this well through the Orphan Well Program using the Landowner Pass-through Payment Agreement. The well was plugged between 3-1-2016 and 3-9-2016 and a plug report is on file in the Division database.

Todays inspection found a new room being built on the north side of the dwelling, in the area of the plugged well. This construction prohibited taking a GPS reading. Due to this construction, there was no restoration required after plugging the well as all areas affected by the well plugging are either within the construction zone or meet restoration requirements. Final Restoration is passed. Pictures were taken today and are located in the Orphan well file for this well.

1686076179 2016-03-04 Plug / Plug Back No Violations On 3/4/2016 the wellhead pressure is at 0 psi. Nicks has assembled a wellhead capable of packing off 1 diameter tubing onto the 2 3/8 tubing. Nicks ran 33 joints of 1 tubing into the well to 825. The well was packed in and plumbed to the steel pit that was on location. Nicks cementers rigged up to the wellhead and began to circulate 12 BBLs of fresh water to condition the wellbore. Once the wellbore was condition, they began to pump 42 sacks of Class A cement that scaled at 14.8 ppg. The cement slurry was pumped at a rate of 1 BBL per minute at 500 psi. The slurry was displaced with .57 BBLs of fresh water. Nicks removed 12 joints of 1 tubing from the well. The cementers rigged back up to the tubing, broke circulation and pumped 42 sacks of Class A cement that scaled at 14.8 ppg. The slurry was displaced with .24 BBLs of fresh water. Nicks then removed the tubing from the well and shut the well in until 3/7/2016.
