0218185432 2015-09-22 Production Wells Violations Noted I was on site to conduct a order follow-up inspection on this well. The last reported commercial production for this well was in 2011. The well is equipped with a pump jack and the valves were open. The well is also being used for domestic gas. The stuffing box around the wellhead was oil stained and covered in parrafin. There was oil staining around the wellhead that measured 5 feet by 8 feet. The tank battery consisted of one 100 BBL production tank and a mechanical separator. There was oil stained soil inside the tank battery dike and under the mechanical separator but was not visible outside of the dike. The tank battery, wellhead and lease road were very overgrown with vegetation. There was no signage associated with this well. The following needs completed by:

1) Clean-up and remove any contaminated oil stained soil inside of the tank battery and around the wellhead (11-06-15) 2) Remove overgrown vegetation around the wellhead, tank battery and lease access (11-06-15) 3) Identification sign posted (10-20-15)

0633643835 2018-08-22 Production Wells No Violations I conducted a compliance notice follow-up for compliance notice 218185432. At the time of this inspection, the well was equipped with a pumping unit and the valve on the tubing flow line was open. There was a domestic line plumbed off of the 4.5 inch casing with a closed valve. The stuffing box around the wellhead was still oil stained and covered in parrafin. There was oil staining around the wellhead still has not been cleaned up. The oil stained soil inside the tank battery dike and under the mechanical separator has not been cleaned up and remedied. The well location was very overgrown with weeds and phragmites. The tank battery consisted of one 100 BBL tank and a mechanical separator. Valves on the separator were open. The diked tank battery did not have legible signage and was very overgrown with phragmites and wetland vegetation. Lease access was stable.
0696528938 2016-01-15 Production Wells No Violations I was on site to conduct an inspection of this well. There have been no changes since the last report dated 11/19/2015.
1447960414 2015-11-06 Production Wells No Violations I was on site for a follow-up inspection to the compliance notice issued on 9/22/2015 for this well. No changes have occurred since the last inspection. I spoke with Charlie Moseo owner of Plateau Energy Corp about when the well and tank battery clean-up efforts would begin. He indicated that he would be on site the following week to begin addressing the compliance issues.
1521137219 2018-02-14 Production Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-202 for failure to maintain liability insurance. At the time of this inspection, but the well was not being produced commercially based on the closed flow-line connections. The was a pump jack unit and associated tank battery (comprised of one 100-barrel steel production tank with mechanical separator). The well and battery are located in an area of dense over-growth of tall grasses and wetlands vegetation. Elements of the system appeared to be in disrepair and the flow-line connections were closed. The had been previously-noted releases of oil from the wellhead and tank battery. The surface areas were obscured by snow cover and not available for visible assessment. There remained no active liability insurance on file for Plateau Energy Corp.
1553533538 2019-03-25 Production Wells No Violations I conducted a compliance notice follow--barrel production tank, and mechanical separator. Valves were shut at separator. No visible oil staining or contamination was observed inside of tank battery that was previously stated in other reports. Tank battery is overgrown with swap grass. No identification was posted. Lease road in fair condition.
