Our inspection found this well to be equipped with an electrically-driven pumpjack. The polish rod was not lubricated. Fresh oil measuring 88 inches by 77 inches surrounded the wellhead, and there was active bubbling through the oil at time of our inspection. The tubing valve coming out of the wellhead was open. We were unable to determine if the production casing valves were open at the time of our inspection because they were coated with fresh oil. 1435082140 | 2015-06-23 | Production Wells | No Violations | I conducted a compliance follow-up inspection of the well and tank battery. The belt guard had been installed on the pump jack. The 8 and 5/8 casing head was exposed so that the source of the leak could be determined, but the well appears to still be leaking as there was active bubbling coming up through the standing water at the well head. The oil-stained soil and previous fresh oil had been removed, however, there was new, fresh oil accumulating in the standing water around the well head. Inspection of the tank battery showed that the unused production tank located north east of the dike, the horizontal separator, and leaking heater treater inside of the tank battery had been removed. The tank battery now consist of one, 210 barrel tank. The brine and oil impacted soil had been excavated out and removed. Construction of a new dike was started, but appears to be unfinished as there are breached and incomplete areas of the dike. No identification was posted at the tank battery. |

Lakeside dug around the well head to determine the source of the leak. A triangle of cement, that was evident around the well prior to excavation, has been totally exposed. The only casing that is visible under the exposed triangle of cement is conductor casing.
Lakeside had removed the contaminated dirt around the well head and the pit around the well is fairly clean of oil. The contaminated dirt is in a stockpile next to the well and Lakeside would like to mix it with manure and spread it near the well. -? leak is still not evident due to the cement triangle being intact, except for the hole at the bottom of the triangle right above the conductor casing. This hole is on the left side of the triangle (s.) if you are facing the pump jack. This is also the side where the gas was bubbling prior to excavation. Further removal of the cement triangle to determine the source of the oil/gas leak will take place next week. |

The well was pumped for 2 hours on by-pass and the 4.5 casing values were shut there was 72 psi. on the casing. There was no oil or gas seen coming from the 8.625. Dave field man for G&O will talk to his supervisor to see what they are gone to do to repair this problem. |

Sutton Pump and Supply will be on location to steam tubing and remove the oil and paraffin from location. |

To discuss the remove of a concrete pad around the wellhead. The well is leaking some oil and gas it looks to be coming from between the 8.625 casing hanger and the 4.5casing. They pumped the well and it showed some oil coming from the annuls. The 4.5 casing pressured up to 80 psi. The pump was shut off. The pressure did not drop to zero but stayed at 70 psi. 4.5 casing integrity looks good at this time. The decision was made to leave the concrete around the well until upper management returns from vacation. |
