2828470 Primary Facility 2019-01-17 Drilling/Alteration No Violations Noted Arrived on location at 09:30. Weather: Clear, cold ~ 18 degs. Onsite to perform routine inspection of (3) recently spud wells on pad. Upon inspection, (3) new 20" conductors, 16" mouse-holes and precast cellar units have been installed for the SV 3H, 22H & 23H wells. One additional well the SV 1H, which was previously in production, has been temp. shut in and disconnected from production unit. All production piping has been removed.
  Liner, drill mats, mud tanks and containment have been installed on location in preparation for drilling of (3) new wells. 
  Overall, wells and location were found in good condition with no obvious concerns or violations noted. Left location at 10:30.

2851558 Primary Facility 2019-02-08 Drilling/Alteration No Violations Noted I arrived at the site at 12:57. The weather conditions are partly cloudy skies and breezy, with temperatures in the mid to upper 40's. The site is quite muddy due to recent snowmelt and precipitation events. Patterson UTI Drilling Rig # 334 is at the location. CHK submitted a Drilling Resumption After Break of 30 or more notification to the Department on 1/25/19. The notice indicated that CHK would commence drilling of the Slumber Valley 3H well on 1/26/19 at 5 AM. I met with Will Doby, site company man, and consultant of CHK. Mr. Doby provided a description of site operations. He stated the following. "The 3H well is done. Cementing of the production casing was completed at 9AM today, 2/8/18. The cement head is rotated. A total of (130) barrels of cement have returned to the surface. The well was drilled to a total depth of (14,571) feet. We are cleaning out the mud pits now. The fluid in the mud pits will be sent to the CHK Sharer site. It is a 50/50 mix of oil and water based drilling mud in
