2217534 Primary Facility 2013-10-25 Routine/Complete Inspection No Violations Noted Mr. Tripp informed me of a (3-4) barrel capacity spill that occurred at the site on 10/24/13 between 11-12 PM. Overpressurization resulted in a loss of drilling fluid out of the well. The loss of the drilling fluid occurred between the main shakers and drying shakers. The fluid was intended to be deposited into a storage container (drill cuttings bin) on containment. The fluid flowed under the bin and spread across the containment. A portion of the fluid escaped containment and impacted the ground surface from the edge of the drilling rig secondary containment berm to the base of the site perimeter earthen berm. Some of the fluid pooled next to a compressor and a tote on a pallet storing equipment and the secondary containment for the production equipment. Pelcatch, otherwise known as suck it up, was used to absorb the fluid on the ground surface. The only additive used during drilling of the surface string is a gel consiting of clay and gel sweeps are used to get cuttings out of the hole. Most of the spill occured on containment.
2233109 Primary Facility 2013-12-30 Routine/Complete Inspection No Violations Noted Flowback procedures were in progress for the 2H well. Mr. Daugherty stated flowback commenced on 12/27/13. Stimulation of the wells was completed about 2-3 weeks ago according to Mr. Berkey. The 1H well is in production. The production secondary containment system and associated equipment has not been completely set up. I observed several petroleum like rainbow sheens in ponded water on the secondary containment liner in a relatively close proximity to the flowback separation equipment and the wellheads. Mr. Berkey ensured me the water on containment from the recent rain event will be removed from the site including the sheens. At the end of the insepction Mr. Daugherty stated a vacuum truck is en route to the site to vacuum the water on containment. I observed some areas of compromised liner integrity, but overall the liner appeared to be in relatively good condition following well stimulation procedures. There were no elevated conductivity readings of concern in any areas in standing water on and off containment.
