2703210 Primary Facility 2018-02-22 Compliance Evaluation In Compliance with Schedule Arrived onsite at 15:15. Onsite to check on progress of re plumbing of all (4) wells annular vents to surface in preparation for long term venting phase as discussed and agreed to in the Depts. meeting with Chief O&G of 2/14/18. All (4) wells, the Polovitch 1H, 1V, 4H and 5H, have had their (20" x 13 3/8"), (13 3/8" x 9 5/8") and (9 5/8" / WB) annular vents plumbed to surface via 2" vents. Vents had been fitted with ball valves and Tees w/ needle valves to access vents for sampling and eventual PBUTs. The Dept. will be meeting Chief O&G Rep., Jon Hafer back at pad tomorrow morning at 09:30 to obtain gas samples off all annuli, where gas is present, for Isotopic analysis. Chief O&G has had all annular spaces and vents purged of fluid and were shut in earlier today in preparation for tomorrows sampling event. (See attached, "As Plugged" Well Bore Schematics for individual wells for plugging details as of 2/14/18).

2703221 Primary Facility 2018-02-23 Compliance Evaluation In Compliance with Schedule Arrived onsite at 09:00. Upon arrival met up with Chief O&G Rep., Jonathan Hafer and D.E.P. Geologist, Eric Rooney. Onsite to initiate the 1st long term, approx. 90 day, venting period as agreed to in meeting of 2/14/18 with Chief O&G. All (4) wells annuli vents, the (20" x 13 3/8"), (13 3/8" x 9 5/8") and (9 5/8" / WB) were re-plumbed to surface and shut-in yesterday in preparation for todays sampling. Mr. Hafer explained that he would be taking gas samples off all the wells annular vents to send off for analysis. Following Mr. Hafers collection of annuli samples, I performed gas screenings at all (4) wells annuli using a ALTAIR 5X gas meter calibrated on 2/5/18. Gas was detected at the following wells annuli:

1H: (13 x 9) = 100% LEL, 100% CH4, 4H: (13 x 9) = 100 % LEL, 30 % CH4, 5H: (13 x 9) = 100 % LEL, 8 % CH4, Following sample collection and gas screenings, all (4) wells annuli were opened to vent to atmosphere. This long term venting period will extend for ~ 90 days. Operator will then be performing

2775961 Primary Facility 2018-08-31 Compliance Evaluation In Compliance with Schedule Arrived onsite at 11:00. Upon arrival met with Chief O&G Rep., Jonathan Hafer to conclude the 7 day PBUTs being performed on all (4) wells annuli vents, the (20" x 13 3/8"), (13 3/8" x 9 5/8") and (9 5/8" / WB). This is the 2nd PBUT being performed after (2) full 90 day, long term venting periods. Pre-shut in test gas screenings were performed at all wells annular vents on 8/24/18 by Mr. Hafer using a Rae Systems Multi-Rae gas meter, calibrated that morning. The only annuli where gas was detected are listed below:

1H: (13x9) = 3.1% CH4, 51% LEL 1V: (9/WB) = 1.6% CH4, 36% LEL 4H: (13x9) = 1.9% CH4, 70% LEL 5H: (13x9) = 6.6% CH4, 100% LEL

Today, the ending 7 day PBUT results were: 1H: (20x13)= 0.40 psi, (13x9)= 12.95 psi, (9/WB)= 0.44 psi. 1V: (20x13)= -0.10 psi, (13x9)= -2.0 psi, (9/WB)= 0.45 psi. 4H: (20x13)= 0.10 psi, (13x9)= 11.10 psi, (9/WB)= 0.70 psi. 5H: (20x13)= 0.65 psi, (13x9)= 5.2 psi, (9/WB)= 0.75psi. - Mr. Hafer explained that he would be collecting samples off all the wells annular vents to sen
