2516350 Primary Facility 2016-07-20 Complaint Inspection Pending At the time of inspection the well was drilled and equipped for production. The site appeared to be stable. No observation of accelerated erosion or sedimentation. When looking at the well site from the access road the well heads are located in the center of the well pad and the remaining well production equipment is located on the left side of the well site. The brine tanks, gas production units, dehydration unit and pipeline pig launcher/receiver are all located on the left side of the well site directly adjacent to the cut slope. Three brine tanks were placed inside of a secondary containment apparatus. No sheen was observed in the standing water inside the containment apparatus.

The cut slope bank is steep and approximately 40’ in height. ERT personnel took gas readings near the production equipment, specifically the dehydration system located on site. A 500 gallon opaque plastic tank, which appeared to be attached to the dehydration unit, produced combustible gas readings in excess of 100% LEL. Th

2516366 Primary Facility 2016-07-28 Follow-up Inspection No Violations Noted Follow up inspection to inspection conducted on July 20, 2016, see inspection #2516350 , this inspection is also related to Complaint # 320656.

On July 28, 2016 the Department requested an onsite meeting with the Operator. The Operator agreed to and scheduled the meeting for July 29, 2016. The Operator sent two subcontractors, L. Casaday from Rock Run, and Jim Drinkwine from Danos. Also onsite during the inspection was PADEP WQS S. Steingrabe and PADEP OGI J. Brannan. The Department began by reviewing the complaint and the observations during the 7/20/2016 inspection. The Department informed Mr. Casady and Mr. Drinkwine that combustible gas measurements taken on 7/20/2016 resulted in the detection of combustible gas at levels exceeding 100% LEL. The measurements were observed at the vent on top of a 500 gallon plastic tank located near the dehydration unit. 2521155 Primary Facility 2016-09-21 Routine/Partial Inspection No Violations Noted Follow up inspection to inspection's #2516350, 320656 and 2516366.

At the time of inspection the 500 gallon plastic appeared to be decommissioned. The 500 gallon plastic tank was disconnected from the dehydration unit and was empty. The smaller plastic tank, located inside the dehydration unit secondary containment, was also removed. No standing water observed inside the dehydration unit secondary containment.

The Department notes that the sample analysis and amount of fluid produced from the dehydration unit requested in inspection report 2516366 has not been received.

2541914 Primary Facility 2016-12-02 Routine/Complete Inspection No Violations Noted Well tagged-shut in. Responded to a small release of Ethel Mercaptin which was cleaned up. Well and equipment looked ok. Operator took steps to ensure that no more releases of Ethel Mercaptin would occur.

2544111 Primary Facility 2016-12-02 Incident- Response to Accident or Event Violation(s) Noted Inspection performed in response to Emergency Response Team’s (ERT) request for a Department follow-up to an odor complaint received by ERT on 12/1/2016. NRC Incident Report (#1165394) relating to odor complaint stated a release of 2 ounces of mercaptan (odorant) from the unconventional drill pad due to unknown reasons. Prior to this inspection, Emergency Response Incident Report and Operator stated initial clean-up measures on the day of the incident consisted of the removal of three sealed 5 gallon plastic buckets of impacted soil. Operator stated that Deomer (deodorant) to neutralize the smell of the mercaptan was applied on the impacted area. A plastic mat/tarp was also placed over the impacted area to suppress the odor until remediation of the soil could be conducted the next day.

2544129 Primary Facility 2016-12-02 Administrative/File Review In Compliance with Policy Department called Operator (Vern Griffin) as part of a follow up to a reported odor complaint that ERT reported to be a result of a mercaptan release on 12/1/2016. Operator informed Department that Apex Energy personnel were in the process of performing scheduled maintenance work that involved replacing the manifold that is attached to a 1000 gallon mercaptan tank. At around 11:30am on 12/1/2016 operator personnel damaged the 1/4” line connected to the mercaptan tank. This line had approximately 2-3 ounces of residual mercaptan that was released to the ground. According to Operator, the 1000 gallon tank had less than 30 gallons of mercaptan at the time of the incident. This tank is usually pressurized, however at the time of the incident; the system was shut-in for the schedule maintenance work. This manifold was being replaced to improve operations at the site as the manifold was of a larger size. At the time of inspectionmercaptan spill has been cleaned-up, impacted soils have been removed and placed in seal drums until final disposition. Impacted area regraded with clean gravel. Department did not detect any odors while on site and/or in the vicinity of the site. Site appears stable with no E&S issues in relation to site and/or access road.
