2180108 Primary Facility 2013-07-02 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was on site and placed on containment. Inspection in response to a phone conversation with operator indicating red line changes to the E&S controls located within the limit of disturbance. The Department notes that the Operator has added additional BMPs and rip rap to the fill side of the pad. The Department suggests the Operator continue to inspect the site weekly and after significant precipitation events to ensure stability of the site and the agreed upon changes to the E&S plan achieve complete vegetation.
2189364 Primary Facility 2013-07-18 Drilling/Alteration No Violations Noted 7/18/2013 Keane rig drilling on Guardasoni 1H depth of 2386. Waiting on cementers to cement 85/8" csg.
2194938 Primary Facility 2013-08-06 Drilling/Alteration No Violations Noted 8/6/2013 Moving Sidewinder rig on location.
2195659 Primary Facility 2013-08-12 Routine/Complete Inspection No Violations Noted At the time of inspection the drill rig was on site and placed on containment. The Operator indicated that the current depth of drilling was 4288'. The Erosion and Sedimentation plan was on site and appeared to properly implemented and maintained as outline in the plan. No observation of accelerated erosion or sedimentation in relation the well pad or access road. The pits located on site had the required 2' of freeboard and appeared to be in sound condition. The Department suggests the Operator continue to inspect the site weekly and after significant precipitation events to ensure stability of the site is maintained.
2211060 Primary Facility 2013-10-02 Drilling/Alteration No Violations Noted 10/2/2013 rig gone waiting for stimulation.
2212534 Primary Facility 2013-10-04 Routine/Partial Inspection In Compliance with Policy
2216981 Primary Facility 2013-10-29 Incident- Response to Accident or Event Pending Inspection the result of a 25 gallon spill reported to the Department on 10/28/2013. The Operator reported a release of reuse water spilled onto the gravel well pad. During the onsite inspection the Department observed a stained line surrounding the well pad and drill pit located onsite. The line appeared to be approximately 3' wide at its apparent beginning and approximately 1' wide at its end stretching in a circular pattern approximately 500' in length. At the time of inspection the Operator and Department agreed that the spill appeared to be greater than the original report of 25 gallons. The Operator agreed to recalculate the amount spilled based on the affected area.
2248575 Primary Facility 2014-02-26 Drilling/Alteration No Violations Noted 2/26/2014 Waiting on completion rig.
2272514 Primary Facility 2014-05-21 Routine/Complete Inspection No Violations Noted At the time of inspection well appeared to be drilled, stimulated and equipped for production. The Erosion and Sedimentation Plan was on site. No accelerated erosion or sedimentation observed in relation to the access road or well pad area. The perimeter E&S BMPs appear to properly installed and functioning properly. The Department suggests the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site in maintained.
2433211 Primary Facility 2015-12-15 Routine/Partial Inspection No Violations Noted At the time of inspection the well appeared to be drilled and equipped for production. The brine tanks and gas production units appeared to be in sound condition. No observation of accelerated erosion or sedimentation in relation the well pad or access road. The Department suggest the Operator continue to monitor the site weekly and after significant precipitation events to ensure the stability of the site is maintained.
2522737 Primary Facility 2016-09-27 Routine/Partial Inspection No Violations Noted At time of inspection the well was drilled and equipped for production.
