Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1870447 Primary Facility 2010-03-17 Complaint Inspection Violation(s) Noted The Department conducted an investigation in response to a complaint alleging a discharge of oil from a well into a stream. I met with the complainant and he drove me back to the site. I introduced myself to Timothy Angerman, VP for the Baron Group. Timothy stated that the well was being blown off and, when the tender had returned, the plug on the brine tank had been missing. Timothy then stated that he was going to be in contact with the police to file a report. The plug on the tank was never found and another was put in its place. Operator estimates that around 1-1/2 barrels of fluids were lost. The production fluids that were released traveled down slope through the E&S controls and into a spring located at the left corner of the site
1871246 Primary Facility 2010-03-22 Routine/Complete Inspection Violation(s) Noted The Department conducted a routine inspection in regards to the spill that happened on 3/16/10. The Department was conducting a follow up inspection to look at the spring after a measurable rainfall event. The Department was leaving the site and noticed a sediment laden discharge off site and into a drainage ditch. This sediment laden water discharged into Barren Run on the downstream side of the pipe. The Department contacted Tim Angerman and let him know of the problem. He said it would be taken care of immediately
1874639 Primary Facility 2010-03-31 Compliance Evaluation No Violations Noted The Department conducted a compliance evaluation inspection in regards to the violations written on 3/22/10. The operator had a discharge of sediment laden water into waters of the Commonwealth. The operator placed a straw bale at the crossover drain at the beginning of the access road. No sediment laden discharge was noted at the time of the CEI. This violation has been abated. The Department noted that the operator has significant amounts of water running down the road due to outcropping of springs. This should be monitored for erosion problems.
1874640 Primary Facility 2010-03-31 Compliance Evaluation No Violations Noted The Department conducted a compliance evaluation inspection in regards to the violations written on 3/17/10. The operator called McCutcheon Environmental Clean up. They responded and dug the contaminated dirt up and placed it in covered roll off boxes. The operator also walked the small spring and cleaned up the remaining contaminates from the banks. These contaminates were placed in garbage bags and disposed of accordingly. Tim Angerman states that this spring will be monitored heavily for the next few months. Absorbent pads and booms are still in place downstream and will be changed periodically.
1880841 Primary Facility 2010-04-22 Routine/Complete Inspection No Violations Noted The Department conducted an investigation in relation to the spill that occurred on site. The Department observed little to no sheen in swale. The Department will continue to monitor the stream after rainfall events. The operator continues to place absorbent pads and booms in this swale.
1953674 Primary Facility 2011-02-18 Routine/Complete Inspection No Violations Noted
