Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1723939 Primary Facility 2008-07-18 Routine/Complete Inspection No Violations Noted well staked, site not constructed, e/s plan drawing revised to show BMP's - E/S should minimize acclerated erosion and sedimentation provided plan is properly implemented
1780937 Primary Facility 2009-04-02 Routine/Complete Inspection No Violations Noted Site constructed, drilling had commenced at the time of the inspection, however it was not ongoing at the time of the inspection. Noted that the below-grade pit located onsite had more than the required two-feet of lined freeboard. Noted that the erosion control measures, i.e. the ditches and subsequent sediment pond located beneath the site appeared to be functioning properly in minimizing accelerated erosion & sedimentation. Found one culvert associated with the access road to require additional erosion control BMP's at the outlet to insure erosion & sedimentation is minimized. The site had required a waiver of the 100-ft distance requirement due to proximity to a stream, and the operator is reminded that should changes need to be made to the E&S plan after the permit is approved, the operator is to contact the Department regarding these changes and a revised E&S plan should be submitted for subsequent approval.
1781501 Primary Facility 2009-03-17 Drilling/Alteration No Violations Noted Marcellus Well, drilling tophole, pit e/s ok
1782428 Primary Facility 2009-04-10 Administrative/File Review No Violations Noted permit review
1798283 Primary Facility 2009-03-25 Drilling/Alteration No Violations Noted Marcellus well completed.
1834843 Primary Facility 2009-09-30 Routine/Complete Inspection No Violations Noted MARCELLUS. WELL IS PRODUCING. FRAC PITS ARE EMPTY, ES OK. SITE IS BEING RESTORED
1878186 Primary Facility 2010-04-14 Routine/Complete Inspection No Violations Noted MARCELLUS
2558593 Primary Facility 2017-02-02 Routine/Partial Inspection No Violations Noted At the time of inspection, the well was drilled and equipped for production. The well apparatus and brine tanks appeared to be in sound condition. The well was labeled. No observation of accelerated erosion to sedimentation in relation to the well site or access road.
