2513767 Primary Facility 2016-08-12 Routine/Complete Inspection Violation(s) Noted Department records indicate well was permitted on 5/18/2009 and expired on 5/1/2010. At the time of inspection, site construction appears to have commenced, as indicated by what appears to be an access road and unlined drill pit. Well not drilled and site has not yet been restored. Per current regulations if a well site is constructed and the well is not drilled, well site shall be restored within 30 days after the expiration of the well permit.

Department requests Operator follow-up with Department, in writing upon investigation of reported concerns identifying cause of violations, what Operator has done and will do to correct situation and achieve compliance and schedule/time frame of anticipated compliance activities.

Violations are being issued under 25 Pa. Code § 78.65(2) for Operators failure to restore well site within 30 days of expiration of the permit. Additional violations may be forthcoming pending Departmental investigation. Department will conduct a follow-up inspection.

2519008 Primary Facility 2016-09-09 Routine/Partial Inspection Outstanding Violations - No Viols Req'd Inspection performed as part of a follow-up to 8/12/2016 inspection (Insp.# 2513767 Enf.# 346334). At the time of inspection site restoration work does not appear to have commenced. Department will conduct follow-up inspections to monitor site restoration progress.

At the time of inspection unlined drill pit has been back-filled, site regarded and seeded. Minimum 70% perennial vegetative growth has not been achieved yet. Department recommends Operator continue to monitor site to ensure perennial growth is achieved and site stability is maintained. Department will conduct follow-up inspection to ensure vegetative growth is achieved.
