2315771 Primary Facility 2014-10-07 Incident- Response to Accident or Event Violation(s) Noted Spill reported to Department via a third party responsible for tending the well. During the inspection the Department observed a brine spill of approximately 4 Bbl. Also observed, in close proximity to the brine tanks was an open road side storm water drain. The Department suggests the Operator take immediate actions to prevent the brine from entering the waters of the commonwealth. The Department suggests the Operator take samples of the contents of the brine tanks located onsite, the impacted soil before and after excavation and a background sample to ensure the affected material has been removed.
2324570 Primary Facility 2014-11-21 Incident- Response to Accident or Event Outstanding Violations - No Viols Req'd At the time of inspection, it appears that the Operator has failed to take action to remove the affected material. Dead grass observed leading from the brine tank valve to the storm water drain. The Operator is currently out of compliance with the current spill policy no 15 day response received as of 11/21/2014.

According to the letter the remediation was scheduled to be completed by the first of the year. At the time of inspection the Operator has not taken action to remediate the affected area. Distressed vegetation observed leading from the brine tank valve to the storm water drain.
2335142 Primary Facility 2015-01-12 Follow-up Inspection Outstanding Violations - No Viols Req'd Follow up inspection to spill reported on 10/7/2014. At the time of inspection the Operator appeared to be in the process of excavation of affected material. The Operator has excavated a ~2' deep area in the vicinity of the spill. Also located on site was a roll off box for waste. The Department suggests the Operator continue to follow the Departments Alternative Process clean up criteria for spills greater than 42 gallon or enter Act 2 voluntarily.

The well apparatus and brine tank appeared to be sound condition. Follow up inspection to the spill reported to the Department on 10/7/2014. The area impacted by the release was backfilled and appeared to be restored with growth sufficient to prevent accelerated erosion and sedimentation.
