Present during the inspection was Jon Sassi, Environmental Protection Specialist, and Bill Shreve, consultant and Stone Energy representative. Mr. Shreve is owner of Shreve’s Well Service, which is plugging the Matoushek 1V. A work over rig is on-site. Attached is a copy of the proposed plugging procedure along with a brief narrative of the Cement Bond log interpretation. During the inspection, procedure #5 had just been completed. Currently setting up to conduct procedure #6. Containment of the site appears to be adequate. Mr. Shreve anticipates completing the plugging process sometime next week.

Present during the inspection was Bill Shreve, consultant and Stone Energy representative. Mr. Shreve is owner of Shreve’s Well Service, which is plugging the Matoushek 1V. A work over rig is on-site. Attached is a copy of the proposed plugging procedure along with a brief narrative of the Cement Bond log interpretation. During the inspection, procedure #16 had just been completed. Mr. Shreve believes that they will be able to run a bond log on the 9 5/8 tomorrow. Containment of the site appears to be adequate. Mr. Shreve anticipates completing procedure # 27 on Friday, 8/1/2014. Mr. Shreve will call me on Thursday, July 31, 2014 to facilitate the Department’s anticipated screening of the well on Friday, 8/1/2014, before the metal plague is set.
2311010 Primary Facility 2014-09-25 Site Restoration No Violations Noted Inspection conducted on 9/25/14 of the Stone Energy Matoushek 1V Well Pad. The Matoushek 1V (127-20006) has been drilled then P&A. The inspection was conducted to assess whether the site meets the minimum requirements as defined in Chapter 102.22(C) for permanent stabilization. The Department’s determination is that the site is in fact permanently stabilized.
