The purpose of the visit was to investigate odor complaint. No odors were observed on pad and no activity is currently occurring on the well site. Three wells have been drilled. Highway construction is currently underway adjacent to the well site and may be the source of the odors. Strong odors have been noticed by others north of the well pad while traveling on Candor Road to Route 22 intersection. Erosion and Sedimentation Best Management Practices installed and maintained; including two sediment traps, compost filter socks, interceptor/diversion channels and rock filters. 2692861 Primary Facility 2018-02-06 Incident- Response to Accident or Event No Violations Noted On 2/6/18 an incident inspection was conducted of the Matchett Farm Well Site in response to an operator reported spill that occurred on 2/5/18. The following was observed:

>>The operator reported on 2/5/18 that a hammer joint on a GPU failed resulting in the release of roughly 10 gallons of condensate to the pad surface. This constitutes a violation of 25 Pa. Code § 78.57(a) in that the operator failed to prevent the release of well fluids to the ground. The violation was found to be immediately corrected.

>>Upon inspection it was found that the impacted soils had been excavated and placed in a roll off box. Cleanup was reported as being completed per the approved plan, no evidence was found to the contrary.

2791144 Primary Facility 2018-10-10 Routine/Complete Inspection No Violations Noted On 10/10/2018 at approximately 1145, I conducted a routine inspection of the Matchett Farm well site located in Robinson Township, Washington County. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared to be in good condition.
  • GPUs had secondary containment mechanisms and appeared to be in good condition. It was unknown as to whether or not the secondary containment could hold 110% of the units volume.
  • All wells on site were clearly marked and appeared to be in good condition.
  • Some waste barrels and materials were left on site and should be removed.
  • The site appeared to be stable and no accelerated erosion was observed.

No violations were found during this inspection.

2811676 Primary Facility 2018-11-30 Routine/Complete Inspection No Violations Noted On 11/30/2018 at approximately 8:15, I conducted a site restoration inspection of the Matchett Farm Well Site located in Robinson Township, Washington County to assess site conditions relative to permit termination. The following was observed:
  • The Act 9 sign located at the site entrance was in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared in good condition.
  • Retention basins on site appeared to be functioning as designed.
  • GPUs on site appeared to be in good condition.
  • All wells on site were marked and appeared in good condition.
  • The site appeared to be stable with greater than 70% vegetative growth.

Based on the inspection of the project area, it was determined that all requirements related to permanent stabilization and permit termination had been attained.

I recommend termination of ESX14-125-0064.

No violations were found during this inspection.
