2587967 Primary Facility 2017-04-27 Preoperation Inspection No Violations Noted On Thursday, April 27, 2017, I conducted a pre-operation inspection at the Thai Volcano Well Site located in North Bethlehem Township, Washington County. The inspection was conducted in response to an operator request in accordance with Act 13, Section 3258 (a.1)The Oil and Gas Act. Mr. James Davis, representing Rice Energy, was also present during the inspection.

The Act 9 sign was in place and clearly legible at the site entrance and all required documentation was available on-site. The access road, which was recently treated for dust suppression, and the well pad are in good condition.

Various stages of vegetation were observed throughout the site which was stabilized with a combination of seed, mulch, erosion control blankets and vegetative growth. All perimeter BMPs have been installed and are functioning as designed. The wet pond on the southern end of the site has been competed and remains to be seeded. The seed for the wet ponds has been ordered and is expected to arrive within the week. The

Drilled a 36” wellbore and set 26” conductor casing at 50 ft. on 5/9/17.

2599727 Primary Facility 2017-05-23 Drilling/Alteration No Violations Noted Falcon Drilling Rig #20

Moved onto this well on 5/20/17 and resumed drilling at 62 ft. KB. The 20” casing was set at 332 ft. on 5/21/17 and cemented to surface with 31 bbls returned. The 13 3/8” casing was set at 759 ft. on 5/23/17. The operator’s drilling prognosis expected the Pittsburgh Coal seam at 650 ft. Operator contacted Department while drilling to the 13 3/8” casing point. The operator informed the Department that coal was not found at the expected depth. The operator continued to drill 30 ft. sections. A drilling break was encountered from 717 ft. to 728 ft. and the drill cuttings at that point consisted of gray shale and traces of coal. No other coal was found. The casing was set and cemented to surface with 14 bbls displaced to surface. The cement then fell 129 ft. and the casing was grouted to surface. Currently at 778 ft. and waiting on cement.

Moved onto this well on 8/15/17 and resumed drilling at 6308 ft. KB. KOP was at 7938 ft. EOB was at 8853 ft. MD (7642 ft. TVD). TD was at 13,794 ft. MD (7714 ft. TVD) on 8/17/17. The 5 ½” casing was set at 13,783 ft. Estimated TOC at 1200 ft.

2748923 Primary Facility 2018-07-05 Routine/Complete Inspection No Violations Noted This well has been completed and is producing. All well and site appear to be well maintained. There is some miscellaneous equipment on site.
2759202 Primary Facility 2018-07-24 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Thai Volcano well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The wells were maintained no leaks or fluids around the well head. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.
