Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
2524771 Primary Facility 2016-09-20 Routine/Complete Inspection No Adverse Event or Action Reported Homeowners cleared brush and removed some soil to reveal an old gas (?) well near front corner of the property. Wellhead is 11" casing w/ cap plate having 1" & 2 3/8" pipe on top, both w/ ball valves and plugs. Well is ~60 feet from neighboring house to north, ~100 feet from the property owner's house. Columbia Gas detected stray gas and took samples 5/13/16, which indicated the origin of the gas was from a well and not their line. After testing around well, dead spots in yard and Columbia test holes in area, the only place that indicated stray gas was beside a sewer manhole across the street: 10-15% LEL.
2527892 Primary Facility 2016-09-30 Routine/Partial Inspection Adverse Events or Actions Reported Visited site to take gas readings around well and general area. Readings immediately around well were 0% LEL. Took readings at dead spots between well and landowner's brother's gas meter and got some positive readings of 10-20% LEL.Took readings at dead spots in yard which roughly overlay service line to landowner's brother's house and got readings of 5-20% LEL. Took readings beside brother's meter and got sustained readings of 35% gas (100%+ LEL)
2534490 Primary Facility 2016-10-17 Routine/Partial Inspection Adverse Events or Actions Reported Tested for gas @ all dead spots in the yard and near old gas well. Old well tests were all 0% LEL. Got gas readings at nearly all dead spots, ranging from 0-5% gas, 3-30% LEL farther from street to 5-35% gas, 100% LEL closer to road. The brother's meter site tested very low, compared to a high reading on 9/30/16. Area around sewer hole cover across the street tested 5-10% LEL.
2534508 Primary Facility 2016-10-19 Routine/Partial Inspection Adverse Events or Actions Reported Met w/ Columbia Gas representatives, Scott Sabocheck and Eric Peterson at the site to discuss stray gas situation and possible sources. Columbia had previously pressure tested both houses' service lines and found them to be fine. Columbia connected their line locator to pipe at wellhead and walked area. They found two possible lines leading from the wellhead: one leads to back of property along the property line, the second leads directly toward the area of highest stray gas readings in the yard.
2618684 Primary Facility 2017-07-24 Routine/Partial Inspection No Viols - Permanently Shut Down Visit to site revealed the stray gas persists in the yard and across the street and is found by dead patches of vegetation. Some dead spots in yard were visually bubbling gas after recent heavy rains saturated the soil. Vegetation was thick around the wellhead, making it unable to get access for gas readings. However, as in past inspections, the higher gas readings are farther away from the wellhead. Test holes ranged from 2% LEL to 15% methane gas, on both sides of the street.
2653389 Primary Facility 2017-10-25 Preoperation Inspection No Viols - Permanently Shut Down Met with representative from Yost Drilling at site to discuss evaluating well to be vented or plugged.
2653413 Primary Facility 2017-10-27 Preoperation Inspection No Viols - Permanently Shut Down Met with property owners to discuss plans to evaluate well, then vent or plug. Left consent forms to be signed.
2712374 Primary Facility 2018-03-29 Routine/Partial Inspection No Viols - Permanently Shut Down Visited site to check status of wellhead and stray gas. Wellhead is intact and leaking a small amount of gas (gas bubbling from tubing connection, wellhead partially submerged after recent rains). Stray gas is bubbling up in several areas in yard.
2746197 Primary Facility 2018-06-06 Routine/Partial Inspection No Viols - Permanently Shut Down Visited site to briefly check for evidence of continued stray gas. Dead spots persist in yard and are within the same stray gas hot-spot area previously identified. No gas tests were made.
2772857 Primary Facility 2018-09-10 Routine/Partial Inspection No Viols - Permanently Shut Down Visited site after heavy rains to look for stray gas bubbling/checking for evidence of continued stray gas. Dead spots persist in yard and are within the same stray gas hot-spot area previously identified. Did gas checks with Bascom Turner meter and confirmed gas expressing from most of the spots. Found gas bubbling in a ditch across the street from the well.

Got signed consent form from owner to allow future work on well.

2806543 Primary Facility 2018-11-26 Routine/Partial Inspection No Viols - Permanently Shut Down Visited site after recent heavy rains to look for stray gas. Limited bubbling in yard and across street in water-filled ditch. Ground is so saturated that gas migration is less than prior visit. Spoke with landowners.
