Drilled 30” wellbore and set 26” conductor casing at 40 ft. on 10/6/16.

2527412 Primary Facility 2016-10-11 Drilling/Alteration No Violations Noted Falcon Drilling Rig #27

Moved onto this well on 10/7/16 and resumed drilling at 40 ft. The 20” casing was set at 328 ft. on 10/9/16 and cemented to surface with 25 bbls returned. Operator drilled 5 ft. of new formation under the 20” casing shoe and no water was observed. The 13 3/8” casing was set at 393 ft. on 10/10/16 and grouted to surface above the basket. Coal was found from 353 ft. to 359 ft. A basket was set at 350 ft. Currently at 409 ft. and TIH to resume drilling.

Note: There is an Alternate Method approved for this well pad. The operator is setting the 20” water protective shallower that the required depth since it is close to the coal. The operator is required to have a dry shoe on the 20” casing prior to drilling ahead.

The 9 5/8” casing was set at 1945 ft. on 10/12/16 and cemented to surface with 20 bbls returned. TD was at 6420 ft. on 10/16/16.

Moved onto this well and resumed drilling at 6435 ft. on 1/24/17. KOP was at 6712 ft. Currently at 7418 ft. MD and building curve.

EOB was at 7850 ft. MD (7494 ft. TVD). TD was at 11,539 ft. MD (7571 ft. TVD) on 1/26/17. The 5 ½” casing was set at 11,539 ft. Estimated TOC is at 6612 ft.

2650557 Primary Facility 2017-10-23 Routine/Partial Inspection No Violations Noted On 10/20/17 a routine partial inspection of the Marchezak John well site was conducted in response to an operator request to review potential site modifications prior to NOT submittal. The following was observed:

>>A landowner requested permission to add fencing around a small grassy area near the site entrance for potential future agricultural use. No earth disturbance was necessary for fencing implementation, use of the area for livestock may result in substantial erosion an is not recommended.

>>An area near the north western corner of the well pad near an area shared by an EQT pipeline showed some third party use damage. The operator reported the seeding was planned to be conducted shortly.

>>Third party access to a nearby conventional well has resulted in an area of exposed soil directly adjacent to the site access road. Reinforcement or remediation was recommended to prevent further damage. The operator request for minor modification approval to add a small patch of stone and gravel to the be

2663864 Primary Facility 2017-11-20 Site Restoration No Violations Noted On 11/20/17 a restoration inspection of the Marchezak John well site was conducted accompanied by Range Resource's Nick Reda in response to an NOT request. The following was observed:

>>Gravel was added to the junction with the access road to a nearby conventional well as discussed during a previous inspection.

>>A newer EQT ESCGP permit overlays the Marchezak permit along one side of the pad. This area does not meet NOT requirements but does not prevent site NOT. The EQT permit is for the NIMI-S007 pipeline.

>>A small disturbed area remained where an unknown third party accessed the site. The area was stabilized and seeded.

Conclusion: The site appears to meet conditions for permit termination.

2741418 Primary Facility 2018-06-14 Routine/Partial Inspection No Violations Noted On 6/14/2018, a routine partial inspection was conducted of the Marchezak John well site along with DEP Water Quality Specialist Adam Ofstun in relation to an inspection of an adjacent pipeline. The following was observed:

>> Site appeared in generally good condition with no visible spill or erosion noted. Four wells observed at time of inspection.

2825496 Primary Facility 2019-01-11 Compliance Evaluation No Violations Noted On 1/11/19 a compliance evaluation inspection of the Marchezak John Site was conducted in response to violations noted on 10/10/17. The following was observed:

>>No residual impact was found in the area of the spill.

>>The site was found to be in good condition with no violations noted.
