2587964 Primary Facility 2017-04-27 Preoperation Inspection No Violations Noted On Thursday, April 27, 2017, I conducted a pre-operation inspection at the Thai Volcano Well Site located in North Bethlehem Township, Washington County. The inspection was conducted in response to an operator request in accordance with Act 13, Section 3258 (a.1)The Oil and Gas Act. Mr. James Davis, representing Rice Energy, was also present during the inspection.

The Act 9 sign was in place and clearly legible at the site entrance and all required documentation was available on-site. The access road, which was recently treated for dust suppression, and the well pad are in good condition.

Various stages of vegetation were observed throughout the site which was stabilized with a combination of seed, mulch, erosion control blankets and vegetative growth. All perimeter BMPs have been installed and are functioning as designed. The wet pond on the southern end of the site has been competed and remains to be seeded. The seed for the wet ponds has been ordered and is expected to arrive within the week. The

Moved onto this well on 5/31/17 and resumed drilling at 62 ft. KB on 6/1/17. Currently at 348 ft. and running 20” casing.

The 20” casing was set at 333 ft. on 6/1/17 and cemented to surface with 21 bbls returned. The 13 3/8” casing was set at 704 ft. on 6/3/17 and grouted to surface above a basket. Coal was found from 659 ft. to 672 ft. A basket was set at 639 ft. The 9 5/8” casing was set at 2448 ft. on 6/5/17 and cemented to surface with 36 bbls returned. KOP was at 2700 ft. TD was at 6573 ft. MD (6243 ft. TVD) on 6/10/17.

Moved onto this well on 8/19/17 and resumed drilling at 6586 ft. KOP was at 7630 ft. EOB was at 8777 ft. MD (7670 ft. TVD). TD was at 13,417 ft. MD (7710 ft. TVD) on 8/23/17. The 5 ½” casing was set at 13,405 ft. Estimated TOC at 1625 ft.

2748916 Primary Facility 2018-07-05 Routine/Complete Inspection No Violations Noted This well has been completed and is producing. All well and site appear to be well maintained. There is some miscellaneous equipment on site.
2759200 Primary Facility 2018-07-24 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Thai Volcano well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The wells were maintained no leaks or fluids around the well head. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.
