2582637 Primary Facility 2017-04-13 Preoperation Inspection No Violations Noted On Wednesday, April 12, 2017, I conducted a pre-operational inspection at the Bovinator Well Site located in Amwell Township, Washington County. The inspection was performed following an operator request in accordance with 58 Pa. C.S. Section 3258 (a.1). Mr. James Davis, representing Rice Energy (Rice), was also present during the inspection.

The Act 9 sign was in place and clearly legible at the site entrance and all required documentation was present on-site. The access road and well pad were in good condition. The stream crossing was in good condition and constructed according to the plan. At the time of the inspection, Rice was in the process of installing concrete safety barriers along the access road in the vicinity of the stream crossing for driver safety during inclement weather which will further protect the stream as well.

All perimeter erosion and sedimentation controls have been installed according to plan and are functioning as intended. The operator should continue their periodic monit

2601690 Primary Facility 2017-06-07 Drilling/Alteration No Violations Noted Butch’s Rathole Service auger rig

Drilled a 36” wellbore and set 26” conductor casing at 50 ft. on 6/5/17.

2666354 Primary Facility 2017-11-30 Drilling/Alteration No Violations Noted Falcon Drilling Rig #29 moved onto this well on 11/27/17 and resumed drilling at 52 ft. KB on 11/28/17. The 20" casing was set at 335 ft. on 11/29/17 and cemented to surface with 24 bbls returned. Currently at 458 ft. and changing air head rubbers. Flow line clogged with drill cuttings.
2670512 Primary Facility 2017-12-13 Drilling/Alteration No Violations Noted Falcon Drilling Rig #29 The 13 3/8" casing was set at 580 ft. on 12/7/17 and grouted to surface above a basket. Coal was found from 533 ft. to 539 ft. Two baskets we placed together at 470 ft.

Note: The operator encountered gas and flared at 433 ft. There was a larger influx of gas when drilling through the coal seam. After consulting with the Department the operator set a packer in the 13 3/8" casing from 481 ft. to 491 ft.

The 9 5/8" casing was set at 2330 ft. on 12/11/17 and cemented to surface with 36 bbls displaced. Currently at 3213 ft. and drilling ahead.

2728618 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Bovinator well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The site is not producing at this time. The wells were spudded and covered with a steel plate. No production equipment onsite.

2768690 Primary Facility 2018-08-29 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349 moved onto this well on 8/27/18 and resumed drilling at 6131 ft. KB. KOP was at 6131 ft. Currently at 7103 ft. and building curve.
2772172 Primary Facility 2018-09-07 Drilling/Alteration No Violations Noted Patterson-UTI Drilling Rig #349. EOB was at 7887 ft. MD (7469 ft. TVD). TD was at 12,787 ft. MD (7529 ft. TVD) on 8/31/18. The 5 1/2" casing was set at 12,776 ft. Estimated TOC at 4346 ft.
