TD was at 18,961 ft. MD (7707.86 ft. TVD) on 5/13/16. The 5 ½” casing was set at 18,951 ft. Operator planned to cement casing to surface. Fluid was circulated to surface but not cement. Estimated TOC is at 1460 ft.

2550844 Primary Facility 2017-01-01 Drilling/Alteration No Violations Noted The department was called out by PEMA for a fire on the Rice Papa Bear pad at approximately 16:03 on January 1st, 2017. Upon arrival at well site at approximately 17:50 the majority of the fire had been extinguished using firefighting foam. Foam was still being applied and used to extinguish truck tires that had not yet been fully extinguished. The department conducted an inspection of fracing equipment, wellheads and the containment on the pad. The fire appeared to be limited to the area around the frac pumping truck units and associated equipment. Several pump trucks were severely damaged with other equipment requiring further inspection to determine damage and safe operation ability.
2551824 Primary Facility 2017-01-03 Incident- Response to Accident or Event No Violations Noted On January 3, 2017, I inspected the Papa Bear well site accompanied by OGI Supervisor Scott Sabocheck, OGI Eric Peterson, and OGI John Rushin as well as a number of Rice Energy representatives in response to a fire and subsequent loss of brine and other fluids to the pad that occurred on 1/1/17. Observations from the inspection are below.

•At the stream crossing on the site access road substantial amounts of gravel and mud had been sprayed over the CFS and into the immediate area of the stream. The issue was communicated to site personnel at the time of inspection and immediate actions were taken to remediate. No violation at this time. •The fire and associated efforts to extinguish it resulted in currently unknown amounts of brine, oil, firefighting chemicals, and other pollutants reaching the surface of the pad via numerous holes burned in the liner. This constitutes a violation of 25 Pa. Code § 78.57(a) and 25 Pa. Code § 78.54 in that production fluids were spilled to the ground. •At the time of inspe

>The site appeared to be in generally good condition with no visible residual impacts from the fire.

2616625 Primary Facility 2017-07-07 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected On 7/7/17 an INCDT inspection of the Papa Bear site was conducted in response to an operator reported spill of 25-30 gallons of produced water on 7/6/17 (CTS ID: 328060). The following was observed:

>>The pull offs near the stream crossing on the site access road noted during previous inspections had been removed and vegetated as reported by the operator.

>>The operator reported that while servicing well equipment an undrained section of pipe leaked 25-30 gallons of production water to the pad. Excavation of the impacted materials had been completed at the time of inspection with no further indication of contamination. This constitutes a violation of 25 Pa. Code 78.54 and 25 Pa. Code 78.57(a) in that the operator failed to contain production fluids.

>>Servicing of site equipment was ongoing at the time of inspection.
