2695034	Primary Facility	2018-02-08	Routine/Complete Inspection	No Violations Noted	On February 8, 2018, the Department performed an inspection of the SWN Bernard McCulley well pad site. The site is located in West Finley Twp., Washington Co. The inspection was routine inspection of the site after hydraulic fracturing operations on the site. The Act 9 and well permit signs were at the entrance to the access road and in good condition. The well pad and access road are in good condition. The well pad and access road are stable with greater than 70% vegetative cover. Two wells on site have been completed and are equipped for production. The operator has recently completed two more wells on site and a service rig was setting up on site to drill out frac plugs in preparation to flow back the two recently drilled wells and place them into production. The 4H well has been permitted but has not been drilled.
2837357	Primary Facility	2019-02-05	Routine/Complete Inspection	No Violations Noted	On 2/5/2019 at approximately 10:30, I conducted a routine site inspection of the Bernard McCulley well site located in West Finley Township, Washington Township. I was accompanied by Bryon Miller (DEP Water Quality Specialist). The following was observed: Act 9 sign located at the site entrance was in good condition. Production equipment appeared to be in good condition. Production fluid tanks within secondary containment appeared to be in good condition. All wells on site were clearly marked and appeared in good condition. The site appeared stable with no signs of accelerated erosion. No violations were observed during this inspection.
