2720040 Primary Facility 2018-04-19 Incident- Response to Accident or Event In Compliance with Policy On 4/19/18 an incident inspection of the Vivian Harbison Well Site was conducted in response to CTS ID: 333587. The following was observed:

>>On 4/13/18 EQT's Mike Cromley reported a ten gallon spill with cleanup beginning immediately as a vacc truck was present on site at the time of the spill.

>>Further communication with operator personnel indicated a spill of 10 gallons of produced water near the offloading point near the production tanks and that the spill cleanup was completed the same day.

>>Site inspection found no visible evidence of contamination, cleanup appears to have been fully completed in compliance with policy.
