2732656 Primary Facility 2018-05-22 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Daniel Petricca West well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed outside of the LOD. The wells were maintained no leaks or fluids around the well head. The production tanks were in good condition no leaks , rust or defects observed. The secondary containment for the site was intact and maintained.

2813613 Primary Facility 2018-12-10 Routine/Complete Inspection No Violations Noted On 12/10/2018 at approximately 09:30, I conducted a site restoration inspection of the Daniel Petricca well site located in Smith Township, Washington County to assess site conditions relative to permit termination. The following was observed:
  • The Act 9 sign located at the site entrance was in good condition.
  • Production fluid/condensate tanks were within secondary containment and appeared in good condition.
  • GPUs appeared to be in good condition.
  • All wells on site were clearly marked and appeared in good condition.
  • The site appeared stable with greater than 70% vegetative cover.
  • Some construction equipment was being stored on site by the landowner.

Based on the inspection of the project area, it appears that all requirements pertaining to permanent stabilization and permit termination had been attained.

I recommend termination of ESX12-125-0079.

No violations were found during this inspection.
