2525236 Primary Facility 2016-04-29 Complaint Inspection No Violations Noted On 4/29/16, I performed a complaint inspection (complaint ID 319254) related to the Lusk site and made the following observations:

•The complainant reported a large increase in water entering their property from the street and from a drain that runs from the Lusk site entrance, under the road, and into the complainant’s yard. •Erosion damage was observed running from the drain pipe, across the complainant’s property, and eventually forming or entering what appears to be a shallow stream bed. •A follow up inspection to verify that site construction is consistent with approved plans is necessary.

2525743 Primary Facility 2016-08-25 Follow-up Inspection No Violations Noted On 8/25/16, I performed a follow up inspection at the Lusk in response to issues found relating to complaint 319254 and made the following observations:

•Significant modifications were found to have been made in the construction of the site entrance without appropriate permits or DEP approval including the following:

     -A gravel pull off was constructed that extends beyond the permitted site limit of disturbance (LOD) and substantially increased impermeable surface directly adjacent to the road.  This construction constitutes a violation of 25 Pa. Code § 102.5(m)(4) for failing to comply with the terms of the permit constructing a major modification without a permit.
    -An unpermitted grated catch basin was installed and connected to the drain crossing under the road which was constructed contrary to permitted site plans and outside the site’s permitted LOD.  This construction constitutes a violation of 25 Pa. Code § 102.5(m)(4) for failing to comply with the terms of the permit constructing a major
2543075 Primary Facility 2016-12-05 Complaint Inspection No Violations Noted On 12/5/16, I performed a complaint inspection at the Lusk well site in response to complaint ID 323917 and made the following observations:

•The complainant reported that a loud alarm was heard Sunday Morning 12/4/16 coming from the site at 7:20am and second time at 7:35. They also reported observing and recording heavy smoke coming from the compressor station on the site. Rice Energy vehicles were described as arriving shortly after. •Inspection of the site found no sign of fire, spill, or other issue. •An adjacent pipeline construction project was being conducted on the pad at the time of inspection. The pipeline employees were unaware of any activity on the site over the weekend. An area below a soil pile from the pipeline construction was noted to be lacking sediment controls, the operator implemented controls upon request. •Subsequent inquiry on 12/5/16 resulted in an official response from the operator that will be included with this report. In summary battery failure on well equipment caused it

2614970 Primary Facility 2017-05-23 Complaint Inspection No Violations Noted On 5/23/27 a COMPL inspection was completed in relation to the Lusk well site in response to complaint 326920. The following was observed:

>>The complainant reported that beginning roughly 18 months prior water had developed an off odor and taste and an intermittent oily film.

>>The complainant also reported heavy dust, increased water entering their basement, green growth on their house, and a number of animal illnesses in the area including blindness developing in two cats, a dog, a horse, and a deer.

>>Complainant expressed concern about activity at both the Blue Moon Compressor Station and the Lusk Well Site.

>>Sample number 3401-057 was collected for 946 and bacteriological analyses.

2643567 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted A RTNC inspection was conducted at the Lusk well site on 10/4/17 in response to an operator request for a meeting to review a proposed minor modification and permitted changes at the well site. The following was observed:

>>A minor modification was requested to move an area to be vegetated from the north end of the pad to the south. The request was accepted.

>>The well site was found to be in generally good condition with four wells observed in operation at the time of inspection.

>>A developed area near the entrance way that was noted to be outside the site LOD and in violation was in the process of being restored. No violation.

>>A drain from the site entrance that previously entered a nearby yard and was previously noted to be outside the site LOD and in violation was in the process of being redirected in accordance with a permitted major modification. No violation.

2681506 Primary Facility 2017-06-16 Administrative/File Review No Violations Noted On 6/16/17 an administrative inspection related to the Lusk Well Pad in response to a complaint (CTS ID: 326920). Sample results for sample 3401-057 collected on 5/23/17 were analyzed for potential oil and gas activity impact. No evidence of impact was found.
2755766 Primary Facility 2018-07-25 Site Restoration No Violations Noted On 7/25/18 a restoration inspection of the Lusk well site was conducted in response to an operator request for permit termination. The following was observed:

>>The site appeared in generally good condition with sufficient stabilizing vegetation throughout.

>>The area and drains near the site entrance that were constructed contrary to permit specifications were in accordance with updated plans.

>>The site appears to meet all conditions for permit termination.
