2578791 Primary Facility 2017-02-28 Routine/Complete Inspection No Violations Noted Today I conducted a routine inspection at the Kearns well pad and the following was observed.

Proper signs were displayed identifying the site. The site was stabilized with thick vegetation no erosion observed. The well heads were painted and maintained no leaks or fluids around well head. The production tanks were in good condition no leaks , rust or defects observed. Secondary containment for the site was intact and maintained.

2765262 Primary Facility 2018-08-17 Routine/Complete Inspection No Violations Noted On 8/17/2018 at approximately 10:15, I conducted a routine site inspection of the Kearns Unit 1H well site located in Hopewell Township, Washington County. The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • Production fluid/condensate tanks were within secondary containment and in good condition.
  • GPUs on site appeared to be in good condition.
  • All wells on site were marked and appeared to be in good condition.
  • Barrels with purge water from monitoring wells and an empty spill waste container were left on site within secondary containment. These should be removed by the operator.
  • A gravel stockpile was left on site which should be removed.
  • The site appeared to be stable with greater than 70% vegetative growth.

No violations were found during this inspection.

2777495 Primary Facility 2018-09-11 Routine/Complete Inspection No Violations Noted On 9/11/2018 at approximately 10:00, I performed a routine inspection of the Kearns Well Site located in Hopewell Township, Washington County in response to a reported slip near the access road following a precipitation event. The slip was brought to my attention by Denny Furmanek (Range Resources). The following was observed:
  • Act 9 sign located at the site entrance was in good condition.
  • A slump of material adjacent to the access road at the site entrance was found and appeared to be contained within the site. The slump was approximately 5 feet in width near the top of the slip and approximately 10 feet in width near the toe. No material from the disturbed material appeared to be leaving the site. The disturbed area should be closely monitored and repaired by the operator.
  • The access road near the site entrance was not in good condition and should be addressed by the operator.
  • All wells on site were marked and appeared to be in good condition.
  • Gas separators on site appeared to be in good co
