2751975 Primary Facility 2018-07-13 Routine/Complete Inspection No Violations Noted This is the fifth and final location in a project, all serviced by the same access road. Construction for all five locations and the road would have been completed in the past several day. The frack pit on this location is holding a small amount of what seems to be water from surface run off. If the water continues to accumulate the operator should have he water pumped out before the pit is used to drill or frack into.
2770924 Primary Facility 2018-09-05 Routine/Complete Inspection No Violations Noted This is the fifth and final location in a project, all serviced by the same access road. Most of the location and access road has been stabilized with stone. An attempt has been made to stabilize the remaining disturbed areas with seed and straw. There, however, has been very little vegetation growth. The permit is clearly posted on a tree near the location.

The operator is currently setting conductor pipe. No drill cuttings have been broadcast past the rig floor. Hay bails near the entrance of this project still need to be removed from the ditch-line. The operator should also consider adding some stone to the out fall of the first cross pipe at the project entrance. No violations are noted at this time.

2771236 Primary Facility 2018-09-06 Drilling/Alteration No Violations Noted No one on site at this time. Drilling rig on location and set up. Drill pipe hanging in wellbore. Drill cuttings and water contained in a temporary drill pit.
2800082 Primary Facility 2018-11-07 Routine/Complete Inspection No Violations Noted This well was recently fracked. The pit is still opened. The pit and the area directly around the well head are covered with a synthetic liner. Most of the fluid has been removed from the pit. There does not appear to have been any spills or unwanted fluid migrations. Lines on the pit liner, marking the varying fluid level, indicate that the minimum free board was met. The contractor should make some added effort to maintain free board space for the remainder of the wells to be fracked in this project.

The access road and location are very saturated and soft. Significant rutting has developed from the heavy truck traffic. Reshaping of the road will be required in some areas. General maintenance, in the way of added stone and/or grading, may be required in others. The well permit is clearly posted near the location.
