Inspection shows Howards on site stimulating the nine stages. An average of 4600gals of fluid and 60sks of sand is being used per stage. Lined containment is placed around the wellhead with fluids contained in a lined pit. While stimulating at 642ft a small push was visible coming out of the 8 5/8 annulus at a nearby Medina well (121-44060). Fluid were contained in a poly tank that was previously tied into. No unusual pressure drop during or after stimulation. During the last stage at 771ft a second push was visible unloading a total of 1700gallons into the tank.

Sediment laden water was leaving the roadway and entering the unnamed trib. Water is continuing to run down the roadway and undermine the filter sock. The roadway is not stable and allowing sediment to enter the waterway.

The previous violation is outstanding.
