Met with Rockdale Marcellus representative Jim Tracey on location at time of inspection. Rig was in the process of drilling the 12 1/4" hole for the intermediate casing at time of inspection. The surface casing was cemented on 6/25/2018. Reviewed the cement job log and no discrepancies were noted at time of inspection. Rig is anticipating cementing the intermediate casing later this evening. Conducted a walk around inspection of location. No violations noted at time of inspection.
2750086 Primary Facility 2018-07-10 Drilling/Alteration No Violations Noted The weather is 85 Deg. F and sunny at the time of inspection. Routine inspection of Rockdale Marcellus LLC., Harer 713 Pad, well # 2H on July 10, 2018.

Finished drilling and cementing 5-1/2" production string on July 9th at 22:07. Skidding over to the Harer 713 4H well on July 10th. Verified cement PTRs (production string), cement samples and driller's log. Rig is estimating to start drilling on then 4H at approximately midnight. No violations are noted at time of inspection. Wellsite looks neat and well organized. DEP Representative - Ulyana Trynovich, Richard Becher, Oil and Gas Inspectors. Rockdale Marcellus LLC. Representative ¿ Danny Brooks, Wellsite Supervisor.

2752945 Primary Facility 2018-07-16 Routine/Complete Inspection No Violations Noted Weather was 88° and sunny at time of inspection. Met with Rockdale Marcellus representative Danny Brooks on location at time of inspection. Rig has moved off this well and is on the 4H. Reviewed the cement job log from the production cement job and no discrepancies were noted at time of inspection. Conducted a walk around inspection of location. No violations noted at time of inspection.
2759790 Primary Facility 2018-08-01 Routine/Complete Inspection No Violations Noted I met with Levi Lee, Levi Haigler, and Clayton Harriman when I arrived. Crews were deploying containment, regrading, and laying rig mats for the upcoming frac (estimated start 8/6). Some containment appeared to be left from the drilling rig, and I observed some small tears at the berms in this area. The Rockwater 43 K barrel MASS was deployed on the site. I observed that there was only 16-18" of freeboard in the primary tank, and that ~6-8" of rainwater had collected in the secondary tank containment. I informed Levi Lee of the containment berm tears and the freeboard and water in the secondary containment at the tank, and said that he would address them.
2763056 Primary Facility 2018-08-07 Incident- Response to Accident or Event No Violations Noted Spill response inspection; 8/7/2018.

On 8/6/2018, Regina Logue (Rockdale) called at 17:15 to report a possible leak in the containment. The estimated volume of the release was 10 gallons. I entered the site at 08:46 and met with Adam Altieri, Jennifer Compton, and Marc McCartor (all representing Rockdale). Water was ponded immediately south of the containment and field conductivity was greater than 20,000 umhos/cm (photo 2). Rig mats were being removed and cleaned from the area. Diesel-based drilling mud was being cleaned from the containment where rig mats were removed. The drill mud appeared to cover most if not all of the containment area where rig-mats were removed.

We walked to the the eastern-most well head (117-21857)and they described the release. It is believed water flowed from the containment surface into the cellar (the cellar was filled with gravel). The containment was not sealed to the cellar, so the water filled the cellar and overtopped. Evidence suggests the produced water flowed from the
