2562144 Primary Facility 2017-02-08 Routine/Complete Inspection No Violations Noted On 2/8/17 the Department of Environmental Protection (DEP) attended a preconstruction meeting for the Johnson K (02-135) well site. Repsol, Brubacher and Larson Design representatives were present.

Larson Design developed the Erosion and Sedimentation (E&S) Control plan. The site will consist of a 400 x 500 well pad, improvement of 4,800 of a private road (William B Street) and construction of 1,112 of new access road. A bioretetion basin, which will also serve as a post-construction stormwater BMP, will be constructed near the northwest corner of the pad. Wetlands and two existing stream crossings are shown on the E&S plan along the private road upgrade from 12+00 - 19+00. The E&S plan notes that the wetlands between these stations are considered Exceptional Value (EV) wetlands. According to the E&S plan, the site drains to an unnamed tributary to Johnson Run, which is classified as a cold water fishery/migratory fishery (CWF/MF). 2579407 Primary Facility 2017-04-04 Preoperation Inspection No Violations Noted On Monday April 3, 2017 Johnny Pazzaglia called to report a ~5 gallon diesel spill near the monitoring well above the pad. I spoke with Woodward and Curran's Jordan Rathbun at the monitoring well (photo) who said an excavator removed the affected soil and contained it yesterday, and that a truck was the source of the diesel that leaked.

Repsol plans to spud the 1st well on this pad this week. I spoke with Johnny Pazzaglia, Repsol, on the phone. He said that they are waiting for the site to dry before they work on it. They plan to mulching/ seeding the slopes next week. A puddle on the pad gave a 0.90 mS/cm w/ an Oakton PCSTestr 35.

2586529 Primary Facility 2017-04-25 Incident- Response to Accident or Event No Violations Noted Pat Bower, Drilling Foreman, said Repsol is drilling the first of 5 Marcellus wells at this site, they are at a depth of about 800'.

Yesterday at 1708 hrs, Repsol called in a diesel fuel release that leaked from the saddle tank of a truck (photo). Randy Fish, Safety, showed me where the release occurred (photo). The fuel tank scraped on the access road, broke its strap, then the back of the tank scraped on the road and developed a breach on the bottom of the tank. Next they pumped out approx. 100 gallons from the 120 gallon tank. They estimate a maximum of 15-20 gallons of fuel released to the road. Bishops excavated the affected access road material, and used some absorbents, which is stored in red rolloff containers on the pad (photo). I used a MiniRAE 3000 PID to screen the area where the diesel leaked, the meter remained at 0.0 ppm the whole time. I did not observe any signs of residual fuel on site.

2596479 Primary Facility 2017-05-23 Routine/Complete Inspection No Violations Noted I arrived on-site @ 13:10 hrs. and met Pat Bauer, Repsol company man. Patterson UTI rig 805 on location; the 04 well had been completed, and Mr. Bauer informed me that they would be spudding the 06 well within the next hour. There were 2 fairly large tears partway up the containment wall in the southwest corner of the pad; Mr. Bauer said he would have them repaired ASAP. I departed @ 14:00 hrs.
2602100 Primary Facility 2017-06-08 Routine/Partial Inspection No Violations Noted I spoke with Patrick Bauer, Foreman, who said they are drilling the 4th of 5 Marcellus wells on this pad. They are drilling with water to the kickoff point. I walked around the site with Randy Fish, Safety. A puddle on the pad gave a reading of 1.3 mS/cm with an Oakton PCSTestr 35.

2606923 Primary Facility 2017-06-22 Routine/Partial Inspection No Violations Noted Tory Rogers (Safety), and I walked the perimeter of the pad, taking several EC readings with an Oakton PCSTestr 35. The highest reading was 1.5 mS/cm from a puddle near the pad entrance (photo).
2621196 Primary Facility 2017-08-01 Routine/Partial Inspection No Violations Noted EC reading of puddle on pad 0.64 mS/cm with an Oakton PCSTestr 35. They moved the drill rig off site. Wooden matting and liner containment remain on the pad. Precision Pipeline is installing the gathering pipeline to this site.
2653628 Primary Facility 2017-10-27 Routine/Complete Inspection No Violations Noted I met with company man Sheridan Anderson and Dave Coxhead on-site. Frac was active; currently zipper fracing the 2H and 4H wells. Sheridan estimated that the frac would be complete by 10/29. The Dyna Tank AST containment was free of water and appeared to be in good condition. Sitewide and secondary containments were also in good condition. Some water was present in various locations on the containment and the pad surface; all showed low conductance (less than .45 mS/cm).
2659859 Primary Facility 2017-11-14 Routine/Partial Inspection No Violations Noted I spoke with Rich Cashen (Repsol) who explained that a service rig with snubbing unit (photo) is drilling out plugs from the hydraulic fracturing job of the 5 Marcellus wells on site.

I got electrical conductivity readings from 3 puddles on the pad with an Oakton PCSTestr 35; the highest was 0.90 mS/cm. JLP

2690523 Primary Facility 2018-02-02 Routine/Partial Inspection No Violations Noted No electrical conductivity readings today, because there is no standing liquid on site. Air temp: 14F. JLP
2713918 Primary Facility 2018-04-03 Routine/Partial Inspection No Violations Noted I took electrical conductivity (EC) readings on site today with an Oakton PCSTestr 35, and a Hanna soil test. A puddle on the pad gave a 0.51 mS/cm reading.

I took electrical conductivity readings from pooled liquid with an Oakton PCSTEstr 35. A puddle near the production tanks returned 0.14 mS/cm. Air temp 45F, sunny. JLP
