2392209 Primary Facility 2015-07-23 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected 1 drilling mud release, 2 flowback releases, 1 hydraulic oil realease reported. Setting up to hydraulically fracture 21H Marcellus well.
2395297 Primary Facility 2015-08-04 Drilling/Alteration No Violations Noted
2671255 Primary Facility 2017-12-14 Routine/Partial Inspection No Violations Noted No electrical conductivity readings from liquid today, as the site is frozen
2702331 Primary Facility 2018-03-07 Routine/Partial Inspection No Violations Noted I got electrical conductivity (EC) readings from liquid pooled on site with an Oakton PCSTestr 35. A puddle on the pad gave a 0.14 mS/cm reading.

SWEPI inspection logbook is in mailbox near production tanks; it lists last inspection on March 4, 2018. Air temp 33F. JLP

2757992 Primary Facility 2018-07-27 Site Restoration No Violations Noted NOT approval.

I took electrical conductivity readings with an Oakton PCSTestr 35. A puddle on the pad was 0.19 mS/cm. JLP

2852336 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Kreitzer 505 Well Site in Rutland Twp, Tioga County on 3/12/2019 at 1200 hours. The weather was partly cloudy and 30 degrees. The act 9 sign was observed at the entrance to the site. The E&S Plan was available for review in the mailbox by the entrance. The Onsite inspection log was in the mailbox by the production tanks. Company personnel last inspected the site on 3/12/2019 noting no issues.

There is currently 4 Marcellus (1H-4H) and 1 Utica (21H) producing wells onsite. The pad is well vegetated and stabilized and no erosional issues were observed onsite today. Containment structures were intact and functioning. All PCSM BMPs were present and functioning properly. There was no evidence of spills or releases onsite today.

No violations or issues noted today, I departed the site at 1245.
