2316981 Primary Facility 2014-10-29 Complaint Inspection No Violations Noted The Department inspected the Watkins 820, permit# ESX11-117-0019, in Chatham Twp., Tioga Co. on 10/29/14 @ 1110 hours in response to a water supply complaint. The weather was 52 degrees and overcast. Present onsite were Jonathan Pennings, DEP WQS and Mike Parker, OSR Swepi LP. No oil and gas activity was taking place onsite during the inspection. A hydro excavator was onsite looking for utility lines buried beneath the pad surface. Mr. Parker stated staged fracking had been completed on the Watkins 820 23H and 25H wells, both Utica wells, between 9/28/14 and 10/18/14. They were hoping to frack the Watkins 21H in the next few days, also a Utica well. He also stated the two Marcellus wells, the Watkins 5H and 5HS were “shut in”. The Watkins 2H through 6H wells were listed as “operator reported not drilled”.
2328155 Primary Facility 2014-12-08 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected The SWEPI, LP Watkins 820 site in Chatham Twp., Tioga Co. was inspected by the Department @ 1230 hours on 12/8/14. The weather was 28 degrees with strong winds out of the southeast. Personnel onsite included Terra Tokarz, DEP WQS, AJ Lewis, onsite OSR, and Matt Bailey, AMEC. Rory Hunter, SWEPI LP contacted the Department at 0930 hours on 12/8/14 to report an environmental cleanup underway on the Watkins 820 pad. Swepi had performed a post-frac screening on the Watkins on 11/17/14. During this screening, soil conductivity readings greater than 1mS/cm were encountered in the vicinity of the tank battery. Field conductivity screenings were not completed until 12/3/14 when the tank battery had been removed, and additional areas of elevated conductivity above 1mS/cm were found. The elevated conductivity identified during post-frac screening located under the tank battery and in the eastern sediment trap puts SWEPI LP in violation of: 78.54 & 301SWMA. Violations are considered immediately corrected.
2484933 Primary Facility 2016-05-17 Routine/Complete Inspection No Violations Noted The SWEPI, LP Watkins 820 site in Chatham Twp., Tioga Co. was inspected by the Department @ 1300 hours on 5/17/16. The weather was 59°, cloudy, winds WSW 8 mph. A valid site ID sign was observed at the entrace to the access road. The PPC Plan, onsite inspection log, well permits and a copy of the E&S plan were observed in the mailbox. The site was last inspected by company personnel on 5/11/2016 with “no change” noted in the comments.

During the onsite inspection, fluid was osberved in the the containment surrounding the 500-barrel- tanks. The fluid was screened for conductivity using an Oakton Multi-Parameter PCSTestr 35. The field conductivity was measured at 2.98 mS/cm. A puddle was observed on the pad surface adjacent to the tank containment. This puddle measured 7.34 mS/cm. An additional puddle (it was not clear in the field if the two were connected) was also screened and the field conductivity measured 6.77 mS/cm. 2494032 Primary Facility 2016-05-17 Administrative/File Review No Violations Noted An administrative report was generated for the SWEPI LP – Watkins 820 site in Chatham Twp., Tioga Co. (ESX11-117-0019). Sample results collected 5/17/2016 indicate the presence of a pollution substance on the ground outside of containment. This constitutes a violation of 25 Pa. Code 78.45 (relating to Oil and Gas Wells) against the Watkins 820 23H well, permit #117-21676, based on the laboratory sample results. In accordance with S 78.54 the well operator shall control and dispose of fluids, residual waste and rill cuttings, including tophole water, brines, drilling fluids, drilling muds, stimulation fluids, well servicing fluids, oil, production fluids and drill cuttings in a manner that prevents pollution of the waters of this Commonwealth and in accordance with §§ 78.55-78.58 and 78.60-78.63 and with the statutes under which this chapter is promulgated.

No violations were cited against eh ESCGP or remaining well permit numbers.

2494159 Primary Facility 2016-06-08 Follow-up Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted an onsite Follow-up inspection at the SWEPI LP – Watkins 820 well site in Chatham Twp., Tioga Co. (ESX11-117-0019) on 6/8/2016 @ 1010 hours. The weather was 50° and raining. The DEP was notified SWEPI would be conducting additional remediation efforts to remove soil with an elevated conductivity related to an unknown/unreported release on 6/8/2016.

Background: A violation of 25 PA. Code 78.54 was issued 5/17/2016 based on sample results, collected from a puddle outside containment by the DEP, showing elevated chlorides and conductivity . On 5/18/2016 SWEPI investigated the DEP’s findings. Mr. Bernethy stated he found areas of elevated conductivity and sent a vacuum truck to the site to clean up the puddles. SWEPI was investigating the secondary containment and the reason no spill was reported.

As of today’s inspection, SWEPI has removed material from the affected area. Approximately ½ a roll-off waste container of material was removed. Please pr
2523518 Primary Facility 2016-09-22 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the Wakins 820 site in Chatham Twp., Tioga Co. on 9/22/2016 @1245 hours. The valid site ID sign is located at the entrance to the access road. The site was last inspected by company personnel on 9/10/2016 with “erosion on lease road. Erosion on back side of pad by frac tanks and gravel.” recorded in the comments section.

No puddles were observed on the pad surface. Standing water was observed in the secondary containment surrounding the roll off tanks. The field conductivity was measured at 800µS/cm. Erosion was observed on the access road as noted in the E&S log. An area on the back side of the tank pad appears to have a mound of sawdust like material. Shredded ECB matting is visible on top of it. No vegetation was observed, but the pile does not appear to be eroding. All E&S controls appear to be functional. No violations are noted at this time. The weather was partly sunny, 30°, fresh snow accumulation of ~3". No activity was observed onsite. No puddles observed on the pad surface. No vegetation was observed, but the site appears to be stable. Erosion and sediment controls were not observed due to snow cover, but no issues were detected. 2722376 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the Watkins 820 Well Site located in Chatham Twp, Tioga Co. on 4/25/2018 @ 1415 hours. The weather was 60 degrees and partly sunny . No activity was observed onsite.

There are five producing wells onsite. Production fluids are contained in Highland tanks. Water was observed in the secondary containment surrounding the Rolloff tank. The water exhibited conductivity within normal range. Two puddles on the pad service were also screened for conductivity and found to be within normal range.

No violations were noted. I departed the site at 1315 hours.
