2194969 Primary Facility 2013-08-12 Routine/Partial Inspection No Violations Noted On August 12, 2013 an inspection was conducted at the Marshlands H Bergey Unit 1 and Marshlands Play, Bergey 812 Pad in Gaines Township, Tioga County. The last inspection conducted on behalf of the operator was on 8/8/13, the only issue noted was that previously noted issues have not been addressed. During this inspection the following was noted: the cut slope above the pad is partially vegetated, but is in need of additional seeding in some areas. Much of the silt sock observed is in need of repair or removal. Further stabilization of the infiltration basin/berm to the southeast of the pad is needed. The site ID sign and permits posted should be reviewed to ensure the information on-site is accurate and up to date. No violations were cited during this inspection.
2461190 Primary Facility 2016-03-02 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Bergey 812 site in Gaines Township, Tioga Co. on 3.2.2016 @ 1130 hours. It was 28°, cloudy with snow flurries. 28 mph winds out of the west. An adequate site ID sign was located at the entrance to the access road. The mailbox was located on the back of site ID sign. The mailbox contained the onsite inspection log and a copy of the E&S plan. The site was last inspected by company personnel on 1/7/2016 with “no change” recorded in the comments section. On 12/22/2015 “areas of slumping on well pad cut slope monitoring” were recorded in the comments section.
