2589343 Primary Facility 2017-04-26 Routine/Complete Inspection No Violations Noted On 4/26/17 at 1446 hrs I conducted a routine inspection at the Endless Mountains Energy State Game Lands 268 well site. It was sunny and 73 degrees F. No one was present during my inspection.

The site consists of the access road, a well pad (325 ft x 450 ft) and an impoundment (130,000 bbl capacity). No wells have been drilled at the site. One well permit, SGL 268 100 1H (117-21589), was issued on 9/6/12. The well was reported as never materialized and the permit expired on 9/6/13; since then, no new well permits have been issued for the site. As per 25 Pa Code Section 78a.65(a)(3), final restoration of the well site should be completed, as no wells have been drilled and there are no active well permits showing intent to drill in the near future.

On 3/18/16, EME provided correspondence stating that the PA Game Commission (PGA) requested that well site restoration be postponed until either expiration of the current lease or until all proposed wells at the site have been completed; a copy of the PGC letter

2728541 Primary Facility 2018-05-10 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the STL resources, LLC SGL268 – B well pad. On May 10, 2018 @1200 hours. The weather was 71 degrees cloudy, with passing rain showers. Winds SSW 11 mph and 61% humidity. The receiving streams are listed as Black Run (EV) and Paint Run (CWF).

Permit ESG29-117-18-0002 was issued to STL Resources, LLC on April 3, 2018 for the purpose of installing a topsoil/compost filter sock infiltration basin, three diversion swales, and two driveway culverts for post-construction stormwater management purposes.

As of today's inspection, no activity was observed. I observed the access road, well pad and impoundment. The impoundment contained water and had adequate freeboard. The site is permanently stabilized as per 25 Pa Code Section 102.22(a).

No violations are noted at this time. Please address any deficiencies noted in this inspection report and contact me with any questions. I departed the site @1330 hours.
