2519011 Primary Facility 2016-09-13 Routine/Complete Inspection No Violations Noted Inspection of SWEPI’s Broadbent 466 pad in Delmar Twp. Tioga County conducted September 13, 2016 at approximately 12:00 pm. There were (3) wells on the pad at the time of inspection; the 5HB (Inactive Status), the 1V (Inactive Status), and the 1V (Plugged). The plugged 1V and the inactive 1V share the same API permit # (117-20737) which records indicate was due to a plug and skid event. At the time of inspection; the 5HB has a production tree in place, assumed to be completed, shut in w/ (2) master gate valves atop production casing spool w/tubing hanger and wing valves at side outlets. There were no pressure gauges on the well, just a taylor plug valve on one of the production casing’s wing valve for a pressure gauge probe or crystal gauge. There was an ESD valve on the proposed line out to prod. equip. with manual lever (not hooked to gas). The cellar was filled with gravel with (4) vent pipes total plumbed to surface. There were (2) 2” vent pipes assumed to be the backside of production casing. One of the
2586159 Primary Facility 2017-04-21 Routine/Complete Inspection No Violations Noted On 4/21/17 at 1047 hrs I conducted a routine inspection at the Broadbent 466 well site. It was overcast and 58 degrees F. No one was present during my inspection. The 1V and 5HB wells have been drilled and were placed on inactive status in 2014. The wells were tagged. No staining or evidence of spills was noted on the pad surface. Interim restoration has been completed and the site is permanently stabilized. Impervious areas have been minimized, temporary E&S controls have been removed and post construction storm water management practices have been installed. The Notice of Termination and Well Site Restoration Reports were approved in 2015. No violations were cited during this inspection. I concluded the inspection at 1220 hrs. TLT
