2359265 Primary Facility 2014-12-29 Administrative/File Review No Violations Noted The DEP received the Remdial Action Plan Completion Report on for a release of produced water spill at the Hege 426 well pad, reported to the PADEP on July 1, 2014. The release was discovered on 7/1/14 within the secondary containment and on the adjacent well pad gravel surface. Standing produced water/fresh water mix was collected by vacuum trucks on 7/1/14 from the secondary containment. Initial soil excavation began on 7/2/14 and was completed, following the removal of frac tanks and secondary containment, on 7/8/14. Twelve confirmation soil samples were collected from the excavation area on 7/8/14. The review will be conducted in accordance with the Alternative Remediation provision of the Department policy document 800-5000-001 Addressing Spills and Releases at Oil and Gas Well Sites or Access Roads.

Interim restoration completed. Pad surface has been vegetated to minimize impervious area. Slopes have been regraded to tie in with the existing contour. Sediment traps and rock-lined channels have been removed. Site is permanently stabilized and temporary E&S controls have been removed. Recommend approval of the Notice of Termination. Restoration Reports acknowledged on 12/21/15. TLT
2586959 Primary Facility 2017-04-25 Routine/Complete Inspection No Violations Noted On 4/25/17 at 1400 hrs I conducted a routine inspection at the Hege 426 well site. It was overcast and 62 degrees F. No one was present during my inspection.

The 1V, 1H - 3H, 5H and 6H wells have been drilled to total depth and are in production, with the exception of the 1V. The 1V well was placed on inactive status on 5/15/14. The wells were tagged. The production equipment has built-in containment, which appeared to be in good condition. No staining or evidence of spills was noted on the pad surface.

Interim restoration has been completed and the site is permanently stabilized. Impervious areas have been minimized, temporary E&S controls have been removed and post-construction storm water management practices have been installed. The Notice of Termination and Well Site Restoration Reports were approved in 2015.

No violations were cited during this inspection. I concluded the inspection at 1545 hrs. TLT

2681918 Primary Facility 2018-01-09 Routine/Complete Inspection No Violations Noted I inspected the SWEPI LP Hege 426 site located in Delmar Twp., Tioga Co. on 1/9/2018 @1325 hours. No one was present during my inspection. There was no activity onsite.

There are 5 wells in production on the pad. The 1H, 2H, 3H, 5H and 6H. The 1V well was drilled to total depth but was place on "inactive" status on 5/15/2014. Well tags were observed on the wells heads. The production tanks onsite consisted of Highland tanks that are self-contained. No staining or corrosion was observed on the tanks. The well site appears to have undergone interim restoration. The well pad has been mostly revegetated to decrease the impervious surface. Post Construction Stormwater Management BMPs appear to have been installed. The NOT and WSRR were approved in 2015.

No issues were noted onsite. No violations are issued at this time. I departed the site @1400 hours.
