2854121 Primary Facility 2019-03-14 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's WOOD 626 Well Pad in Sullivan Twp, Tioga County on 3/14/2019 at 1440 hours. The weather was cloudy and 57 degrees. The E&S Plan was available for review in the mailbox by the act 9 sign at the entrance. The onsite inspection log was found in the mailbox by the production tanks. Company personnel last inspected the site on 3/11/2019, noting no issues.

There are currently four producing wells onsite (1H, 2H, 3H & 6H). Containment structures were intact and functioning. No evidence of leaks, spills, or releases to the pad surface could be found. All PCSM BMPs were installed and functional. Puddles on the pad surface were within normal conductivity ranges.

No violations noted today, I departed the site at 1515.
