2436073 Primary Facility 2015-12-21 Site Restoration No Violations Noted Restoration inspection. Notice of Termination and Well Site Restoration Report received on 12/15/15. Interim restoration completed at site. Well pad minimized and temporary E&S controls removed. Site permanently stabilized with the exception of an erosion channel on the north fill slope. Recommend denial of the Notice of Termination based on presence of the erosion channel. Restoration Report acknowledged on 12/21/15. TLT
2494303 Primary Facility 2016-06-14 Site Restoration No Violations Noted Restoration inspection. Notice of Termination received on 6/13/16. Original NOT was submited on 12/21/15 but was denied due to the presence of an erosion channel. Since then, the erosion channel has been repaired and stabilized. Site has permanent vegetative cover and temporary E&S controls have been removed. The Well Site Restoration Report for the 1V well was received with the original submission of the NOT on 12/21/15 and was previously acknowledged. TLT
2752461 Primary Facility 2018-07-17 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this vertical, completed, unconventional well was conducted July 17, 2018. At the time of inspection, a crew was rigging up for final plug and abandonment. Plugging was scheduled for yesterday but the rig was delayed. I met with the companyman to discuss the well's current condition and plugging procedure. There was reportedly no pressure outside of production casing. The production casing reportedly had 75 PSI and blew down without issue. The operator intends to isolate perfs with a bridge plug, set cement plug in cemented portion of production casing, pull retrievable production casing, set cement plugs across both casing shoes (inside 7" intermediate casing) and top off with gravel to surface on this well. No obvious issues were observed, discussed, or expected at this time.
2754057 Primary Facility 2018-07-20 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location for final plug and abandonment. Wireline on location to set bridge plug. I spoke with the companyman to discuss operations on the pad. They encountered an obstruction at 5200' which has since been cleared and the crew is back on schedule setting bridge plug to isolate perforated interval and set 1200' cement plug on top. The crew will attempt to retrieve production casing on this hole and set remaining cement plugs. No obvious major issues were observed.
2755791 Primary Facility 2018-07-25 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a plugged unconventional well. The rig is off location following final plug and abandonment completed earlier this week. At the time of inspection, the plugged well is buried below ground level. There were no open holes, discharges, venting, or other obvious issues observed at the surface location of the plugged well. No gas was detected with an Altair 5x gas meter in the gravel standing above with well. There was standing water on the pad near the well. No bubbling was observed.
