2434038 Primary Facility 2015-12-14 Site Restoration No Violations Noted The Department inspected the SWEPI LP site the Bauer 849 (ESX10-117-0153) in Middlebury Twp., Tioga Co. on 12/14/2015 @1000 hours, in response to receipt of a Notice of Termination and a Well Site Restoration Report by the NCRO on 12/2/2015. The site was last inspected by company personnel on 12/11/2015 with “no visible issues” recorded in the comments. The well permit was observed in the mailbox. The pad size has been reduced to ~125’ x 150’. The surrounding area is stabilized by at least 70% vegetative cover. A visible tag was observed on the well head. No temporary E&S controls were observed onsite. This inspection meets the requirements of 25 PA Code 102.22. No violations were issued during this inspection. Approval of the Notice of Termination is recommended at this time. The Well Site Restoration Report has been processed. I departed the site at 1100 hours.
