The pad has been partially restored. A Notice of Termination and Well Site Restoration Reports for the site were approved by the Department of Environmental Protection on July 29, 2015 (Inspection Nos. 2396253, 2396262, 2396274, 2396276). No evidence of accelerated erosion or sedimentation was observed during this inspection. No violations were cited.

2752598 Primary Facility 2018-07-17 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine (plugging) inspection of this vertical, completed, unconventional well was conducted July 17, 2018. CC Forbes' workover rig is on location for final plug and abandonment, running in the hole with tubing at the time of my arrival. No companyman was on site at the time of my arrival. I spoke with the plugging contractor to discuss the procedure. A CIBP has been set at 6110' to isolate the perforated interval. No pressure was reportedly observed since setting the bridge plug. The are running back in the hole to the plug in order to load the hole with mud before spotting cement plug. The crew shut down and I checked (3) annular vents for flow. There was no audible or visible venting and no significant pressure felt at the vents, verified with an Altair 5x gas meter. No obvious major issues were observed.
2755885 Primary Facility 2018-07-20 Plugging(Includes Plugged/Mined Through) No Violations Noted The rig is off location following final plug and abandonment operations. At the time of inspection; all casing is left in hole (cemented to surface). A bucket was placed on well on top of open wellhead B-section/tubing hanger. No gas was detected inside of the production casing with an Altair 5x gas meter. No gas detected at the mouth of the annular vents. There were no discharges, venting, or any obvious major issues observed.
2763871 Primary Facility 2018-08-15 Plugging(Includes Plugged/Mined Through) No Violations Noted A routine inspection of this plugged unconventional well was conducted August 15, 2018. The rig is off location following final plug and abandonment and a Certificate of Plugging has been furnished by the operator. The Certificate shows the well to meet the minimum plugging requirements outlined in 25 PA code Ch. 78. At the time of inspection, the plugged well is buried below ground level. There were no open holes, discharges, venting, or obvious issues. No gas was detected with an Altair 5x gas meter at the surface location.
