2183859 Primary Facility 2013-07-12 Routine/Partial Inspection No Violations Noted On July 12, 2013 a routine partial inspection was conducted at the Doan 893 pad in Deerfield Township, Tioga County. The last inspection conducted on behalf of the operator was on 7/9/2013. Two wells have been drilled at this site. Drilling of the 893 1V (117-20700) was competed on July 22, 2010 and drilling of the 870 5H (117-21466) was completed on April 15, 2012. The site has not yet been restored as per the Site Restoration Plan in the ESCGP (Figure 6). This constitutes a violation of Section 3216 of the 2012 Oil and Gas Act. The violation will be cited against the 117-21466 permit (Doan 870 5H). No violations were cited against the ESX10-117-0083 or 117-20700 permits.
2454407 Primary Facility 2016-02-25 Routine/Complete Inspection No Violations Noted The Department of Environmental Proteciton (DEP) inspected the SWEPI LP – Doan 893 well pad in Deerfield Twp., Tioga Co. on 2/25/2016 @ 1230 hours. The weather was 39° and overcast with rain/freezing rain. An adequate site ID sign was observed at the entrance to the access road. A copy of the well permit, E&S plan, and PPC plan was observed in the mailbox. The onsite inspection log shows the site was last inspected by company personnel on 2/10/2016 with “site snow covered” in the comments section. The disturbed acreage is listed as 5.18 acres with the total disturbed area listed as 5.18 acres. The receiving stream is the Cowanesque River/Susquehanna River Basin in Pa (WWF, MF).
2479582 Primary Facility 2016-04-26 Site Restoration No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Doan 893 well pad in Deerfield Twp., Tioga Co. on 4/26/2016 @ 1045 hours in response to receipt of a Notice of Termination. The weather was 59° and overcast, winds WSW 16 mph. An adequate site ID sign was observed at the entrance to the access road. A copy of the well permit, E&S plan, and PPC plan was observed in the mailbox. The onsite inspection log shows the site was last inspected by company personnel on 4/18/2016 with “no change” in the comments section. The disturbed acreage is listed as 5.18 acres with the total disturbed area listed as 5.18 acres. The receiving stream is the Cowanesque River/Susquehanna River Basin in Pa (WWF, MF).

Two wells were observed on the pad surface. The pad measures ~125’ x 150’. The site is mostly level with a perimeter berm. The pad surface appears to have been revegetated and is supporting ~70% cover. The surrounding area is also supporting ~70% vegetative cover. No standing water was observed on
