Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1993399 Primary Facility 2011-08-04 Routine/Complete Inspection No Violations Noted Coil tubing unit on site
2010960 Primary Facility 2011-10-11 Follow-up Inspection No Violations Noted
2377371 Primary Facility 2015-06-09 Site Restoration No Violations Noted Restoration Reports submitted for the 3 Marcellus natural gas wells on site. Grass growing well on restored pad area
2397854 Primary Facility 2015-07-29 Site Restoration No Violations Noted nterim restoration has been implemented on the well pad. A significant portion of the pad surface has been decompacted and vegetated. A total of 112.71 tons of drill cuttings, 475.89 barrells of drilling mud, and 8793.87 barrels of fracing fluid are reported to have been disposed of at appropriate waste disposal facilities. The reported amounts were dived aqueally amongst the three wells.
2668208 Primary Facility 2017-12-06 Routine/Complete Inspection No Violations Noted Inspection to assess overall site conditions. No violations cited.
2668210 Primary Facility 2017-12-07 Routine/Complete Inspection No Violations Noted Inspection to assess overall site conditions. No violations cited.
2725658 Primary Facility 2018-04-16 Routine/Complete Inspection No Violations Noted Routine inspection; 4/17/2018.

At the time of the inspection, three wells were in production and rain was falling steadily. Water was ponding on the pad surface and vegetated pad surface and flowing down the access road. Water flowing down the access road was causing scouring of the top rock. I recommend re-grading the access road and making sure water from the well pad is directed off the access road and into the vegetated area upslope of the basin. No violation noted.
