2487200 Primary Facility 2016-05-19 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Mitchell 456 well pad, located in Jackson Twp, Tioga Co. The weather was partly sunny 65 degrees. A site ID sign was observed at the entrance to access road. A copy of the E&S plan, well permits, and onsite inspection log was located in the mailbox onsite. The site was last inspected by company personnel on 5/12/2016 with the following observations “ 1. no new issues observed 2.beautiful day”. As of today’s inspection all fill slopes appear to be well vegetated. Some areas of the cut slope are not growing >70% vegetative cover, but appear stable. Puddles were observed on the pad surface and field screened for conductivity. Puddles near the permanent containment for the production tanks, in the turnaround area, measured ~1000 µS/cm – 1700 µS/cm. Standing water around the well head and puddles on other areas of the pad measured ~500 µS/cm. All E&S controls appear to be functioning. No violations are noted at this time. I departed the
2746435 Primary Facility 2018-06-26 Site Restoration No Violations Noted The DEP conducted a RESTORATION inspection of the SWEPI LP - Mitchell 456 Well Pad located in Jackson Twp., Tioga Co. on 6/26/2018 @1215 hours. The weather was 75 degrees, humidity 42% winds S 7mph. There was no personnel or activity onsite.

The access road and well pad were observed. Areas surrounding the pad appeared stabilized by perennial vegetation. Some areas of the cut slope were rocky, but stable. 2855020 Primary Facility 2019-03-18 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Mitchell 456 Site in Jackson Twp, Tioga County on 3/18/2019 at 1100 hours. The weather was clear, sunny & 33 degrees. The act 9 sign was observed at the site entrance. The E&S Plan was in the mailbox by the entrance.

There are currently 6 wells in production onsite (1H-6H). The site remains well vegetated, stabilize, and no erosional issues were observed. PCSM BMPs are installed as planned and functioning. Containment structures were intact and functioning. No evidence of leaks, spills, or releases could be found. Puddles on the pad surface were within normal conductivity ranges.

No violations or issues noted. I departed the site at 1150.
