2851945 Primary Facility 2019-03-12 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Burleigh 508 Well Pad in Rutland Twp, Tioga County on 3/12/2019 at 1035 hours. The weather was cloudy and 27 degrees. The act 9 sign was observed at the site entrance. The inspection log was in the mailbox by the gate. Company personnel last inspected the site on 3/11/2019 and noted no issues.

There is currently one producing well onsite (5H). The pad remains well vegetated and stabilized. Containment structures were intact and functioning. There was no evidence of spills or releases or accelerated erosion today. No PCSM BMPs installed due to pad being under 5 acres.

No issues or violations noted today, I departed the site at 1105.
