2211044 Primary Facility 2013-10-07 Routine/Partial Inspection No Violations Noted On October 8, 2013 a routine partial inspection was conducted at the Johnson 435 pad in Shippen Township, Tioga County. The last inspection conducted on behalf of the operator was on 10/7/2013. The cut slope above the pad is in need of further stabilization. No evidence of sediment being transported off-site was observed during this inspection. Drilling of the 1H well was completed in 1/8/2010. 25 PA Code § 78.65(3) – Site Restoration states that within 60 days after restoration of the well site, the operator shall submit a well site restoration report. No violations were cited during this inspection.
2284691 Primary Facility 2014-07-08 Site Restoration No Violations Noted On 7/8/14 an inspection was conducted at the Johnson 435 pad in Shippen Township, Tioga County. The site was determined to meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was observed. The Restoration report has been received by the DEP. No new violations were cited during this inspection.
2397295 Primary Facility 2015-07-29 Site Restoration No Violations Noted The Department inspected the Johnson 435 (59-09-804) on 7/29/2015 @ 1130 hours in response to receipt of a Notice of Termination submitted to the Department on 7/29/2015. An adequate site ID sign was observed at the base of the well pad along the access road. The E&S plan was located in mailbox onsite. Total disturbed area is listed as 71.5 acres. The receiving stream is listed as Darling run/Pine creek gorge (HQ). The company E&S inspection log was also located in the box. The site was last inspected on 7/29/2015; sediment sock was removed on 6/29/2015. This inspection complies with 25 PA Code §102.22 . No violations issued during this inspection. Approval of the Notice of Termination is recommended at this time.
2421300 Primary Facility 2015-11-03 Administrative/File Review No Violations Noted The Department conducted an administrative review of the Johnson 435 (59-09-804) on 11/3/2015 @ 1000 hours in response to receipt of the Well Site Restoration Report for the Johnson 435 1H (API#37-117-20368) by the regional office on July 27, 2015. The Well Site Restoration Report has been reviewed and accepted. This inspection references the attached report dated 7/29/2015, at which time the site was found to comply with 25 PA Code §102.22. No violations are issued at this time.

2642080 Primary Facility 2017-09-29 Routine/Complete Inspection No Violations Noted On 9/29/17 at 1230 hrs I conducted a routine inspection at the Johnson 435 well site. It was mostly sunny and 62 degrees F. No one was present during my inspection.

The 1H well is tagged and in production. The production equipment has built-in containment, which appeared to be in good condition. The pad surface was dry and no staining or evidence of spills was noted.

Interim restoration has been completed and the site is permanently stabilized. No violations were cited during this inspection. I concluded the inspection at 1315 hrs. TLT
