The 4H and 6H wells have been drilled, but are not yet in production and 5H well has a permit, but have not yet been drilled. During this inspection the ground was partially frozen. The perimeter of the site was walked and all erosion control BMPs were observed. There was no evidence of any spills found. Several areas, including the cut slopes of the access road, the fill slopes of the water impoundment and the cut slopes surrounding the pad are not stabilized. These areas must be covered with either a minimum uniform coverage of mulch and seed, with a density capable of resisting accelerated erosion and sedimentation or an acceptable BMP which minimizes accelerated erosion and sedimentation. A follow-up inspection will be conducted to determine if escalated enforcement is necessary. - MRN

2181366 Primary Facility 2013-07-03 Routine/Partial Inspection No Violations Noted On July 3, 2013 a routine partial inspection was conducted at the Hillside 804 pad in Gaines Township, Tioga County. The last inspection conducted by AMEC on behalf of the operator was on 7/2/2013. The permits associated with this site and their status area as follows: 117-20359 (1H - Operator Reported Not Drilled (ORND),117-20360 (2H - ORND),117-20361 (3H - ORND),117-20362 (4H - Inactive), 117-20363 (5H - ORND),117-20464 (6H - Inactive), 117-20948 (1H - ORND), 117-20949 (2H - ORND), 117-20950 (3H - ORND) and 117-20951 (5H - Active). Two wells have been spudded, but neither are in production. The fill slopes of the pad and impoundment are well vegetated and stable. The cut slopes are mostly vegetated but should be monitored until 70% perennial vegetation is established. Puddles on the pad surface were screened and none exhibited elevated conductance. No violations were cited during this inspection.

2522027 Primary Facility 2016-09-14 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Hillside 804 well site (ESG10-117-0019) located in Gaines Twp., Tioga Co. on 9/14/2016 @ 1330 hours. The weather was 75°, sunny.

The site was last inspected by company personnel on 9/8/2016 with “no change” noted in the comments section. On 8/11/2016 Shell noted “erosion channels in several areas on access road”. On 5/12/2016 Shell noted “well site has been reclaimed – veg @ 0%”. The site is located within the McCracken Hollow and Lewis Run watersheds. McCracken Hollow and Lewis Run are both tributaries of Elk Run and thereby Pine Creek. Lewis Run is classified as a HQ watershed with CWF designation. Elk Run is classified as a HQ watershed with CWF designation. Pine Creek is classified as EV.

The pad size currently measures ~300’ x 375’. The smaller tank pad that was located to the NE corner of the site has been restored. The 4H and 6H wells have been drilled and are currently in regulatory inactive status. The fill slope appears

2537272 Primary Facility 2016-11-08 Site Restoration No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Hillside 804 well site (ESG10-117-0019) located in Gaines Twp., Tioga Co. on 11/8/2016 @ 1500 hours. The weather was 60°, partly sunny.

The site was last inspected by company personnel on 11/8/2016 with “no visible issues” noted in the comments section. On 11/2/2016 JC noted “No visible issues. Veg greater than 70%.” McCracken Hollow and Lewis Run are both tributaries of Elk Run and thereby Pine Creek. Lewis Run is classified as a HQ watershed with CWF designation. Elk Run is classified as a HQ watershed with CWF designation. Pine Creek is classified as EV.

The pad size currently measures ~300’ x 375’. The pad appears approximately 1/3 revegetated by volunteer regrowth. The smaller tank pad that was located to the NE corner of the site has been restored. Infiltration berms appear to be installed and stabilized with vegetation. 2645154 Primary Facility 2017-10-10 Routine/Complete Inspection No Violations Noted On 10/10/17 at 0848 hrs I conducted a routine inspection at the Hillside 804 well site. It was mostly sunny and 63 degrees F. No one was present during my inspection.

The site was idle. The 4H and 6H wells were tagged and remain on inactive status. No equipment or materials were staged at the site. No staining or evidence of spills was noted on the pad surface.

An impoundment is located to the south of the well pad fill slope. The impoundment is protected by a fence and was nearing capacity with what appeared to be freshwater. It is recommended that the impoundment be drawn down to provide more freeboard.

Interim restoration has been completed and the site is permanently stabilized. Temporary E&S controls have been removed and post-construction storm water management practices have been installed. The Notice of Termination and Well Site Restoration Reports were approved in 2016.

No violations were cited during this inspection. I concluded the inspection at 1025 hrs. TLT

2659142 Primary Facility 2017-11-09 Plugging(Includes Plugged/Mined Through) No Violations Noted Rig on location, rigged up for plugging of first of 2 uncompleted wells. Other well was capped. There was no Companyman on location. Plugging rig contractor was onsite. The crew was not actively working on well. BOP on the well, tubing with TIW in hole. I asked the plugging rig contractor where they tagged attainable bottom, for which they claimed they did not know.

No valves were manipulated during this inspection.

2660507 Primary Facility 2017-11-15 Plugging(Includes Plugged/Mined Through) No Violations Noted The rig is off location following the plug and abandonment of two uncompleted wells. Both wells were plugged and buried at the time of inspection. I used an altair 5X gas meter to monitor for gas at ground level at both surface locations of the wells. There was no combustible gas detected. No obvious issues were observed.
