2212155 Primary Facility 2013-10-10 Routine/Partial Inspection No Violations Noted On October 10, 2013 an inspection was conducted at the T. Pierson 801 pad in Gaines Township, Tioga County. The last inspection conducted on behalf of the operator was on 10/9/13. The Marshlands Unit 2 well, 2H and 5H wells are drilled; the 1H well permit is active, but the well hasn't been drilled, and the 4H, 6H, 3H, 4H and 6H wells have been reported by the operator as not drilled. The site is stabilized and well vegetated. Several puddles were screened and no exhibited elevated conductance. No issues were noted and no violations were cited during this inspection.
2481571 Primary Facility 2016-05-04 Follow-up Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Pierson 801 (ESG10-117-0009) site in Gaines Twp., Tioga Co. on 5/4/2016 @ 1130 hours, as follow-up to the INCDT inspection conducted 3/22/2016. As noted in inspection record# 2467112, on 3/22/2016, SWEPI had notified the Department they encountered liquid with an elevated conductivity and a strong diesel odor while excavating around the Marshland Unit 2 (MU2) well head to fix a leaking annulus vent. As of today’s inspection, the excavated area around the MU2 well head has been backfilled and re-graded. The roll-off container was still onsite. The pad surface appeared very wet, with numerous puddles. The puddles were screened for field conductivity using an Oakton PCSTestr 35. Most of the puddles screened for field conductivity measured 475-800 µS/cm. Standing water was observed in the secondary containment surrounding the produced water tanks. Sample 4427-106 collected ftom the excavated pit at the toe of the fill slope shows elevated levels of iron (25.5 mg/L), manganese (28.5 mg/L) and a pH of 5.6. Violations are considered resolved, and the status is changed from “PENDING” to “NOVIO”.

2520383 Primary Facility 2016-09-14 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Pierson 801 (ESG10-117-0009) site in Gaines Twp., Tioga Co. on 9/14/2016 @ 1230 hours. The weather was 75 degrees and partly sunny. The onsite inspection log stated the site was last inspected by company personnel on 8/30/2016 with “no visible issues” recorded in the comments section. The inspection on 8/22/2016 noted “veg @ 75% in reclaimed areas”.

As of today’s inspection, no standing water was observed on the pad surface or in secondary containment. Disturbed areas appear to be stabilized with vegetation. All E&S controls appear to be functioning properly. No violations are noted at this time. I departed the site @ 1300 hours.

2536513 Primary Facility 2016-11-08 Site Restoration No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Pierson 801 (ESG10-117-0009) site in Gaines Twp., Tioga Co. on 11/8/2016 @ 1300 hours, in response to receipt of a Notice of Termination received by the DEP on 10/1/2016. The onsite inspection log stated the site was last inspected by company personnel on 10/31/2016 with “no visible issues, construction on the pad” recorded in the comments section. The inspection on 10/18/2016 noted “reclaim nearly 100% regrowth”.

As of today’s inspection, standing water was observed on the pad surface with a field measured conductivity between 480-600 µS/cm. Standing water inside the production tank containment had a field measured conductivity of 1980 µS/cm. The newly installed infiltration berm below the SW fill slope appears to be stabilized by at least 70% vegetative cover. All cut and fill slopes appear adequately vegetated and stabilized.
