The site ID sign was present and E&S control plan was on-site. The last inspection conducted on behalf of Novus was on 5/7/12. No erosion issues were observed. All areas of the pad are well vegetated and stabilized. Several puddles on the pad surface were monitored for elevated conductance. No elevated readings were found. The 1H well is the only permitted well on this site. Section 3216 of 58 Pa. C.S. §§ 3201 - 3274 (relating to Development) ("2012 Oil and Gas Act") requires an operator to restore the well site within 9 months after completion of drilling of a well. Consideration should be given to restoring the site or requesting an extension under the provisions of Section 3216(g). No new violations were cited during this inspection. - MRN

2315970 Primary Facility 2014-10-15 Routine/Complete Inspection No Violations Noted The Department inspected the Tenaska, Inc. Brookfield well pad in Brookfield Twp., Tioga Co. on 10/15/14 @ 1230 hours. The weather was 66° rain with 93% humidity and wind gusts of 6mph. An adequate site ID sign was observed at the entrance. A well tag was observed on the Brookfield 1H well head, permit# 37-117-20208. The pad surface measured 250’ x 150’. Approximately 150’ x 100’ of the pad surface had been seeded and was growing vegetation. This area had not been re-graded before being seeded. The cut slope contained ~70% vegetation. The fill slopes were fully vegetated and appeared stable. According to onsite inspection logs the site was last inspected on 3/1/2014. Puddles were observed on the pad surface and the conductivity was measured at 175 µs/cm. The containment around the produced fluid tanks contained standing water, but was not accessible due to gates. No violations were noted during the inspection. I departed the site @ 1300 hours.

A well tag was observed on the Brookfield 1H well head, permit# 37-117-20208. The pad surface measured 250’ x 150’. Approximately 150’ x 100’ of the pad surface had been seeded and was growing vegetation. This area had not been re-graded before being seeded. The cut slope contained ~70% vegetation. A gully is forming down the south face of the fill slope. Water appears to be channelizing on the pad surface and down cutting through the fill slope.(please see attached pictures). Silt sock is visible along the eastern and southern sides of the pad. The containment around the produced fluid tanks contained standing water, but was not accessible due to gates. No violations were noted during the inspection. I departed the site @ 1230 hours.
2549969 Primary Facility 2017-01-04 Routine/Complete Inspection No Violations Noted Routine inspection conducted at the Tenaska Resources - Brookfield 1 well site located in Brookfield Township Tioga County. The weather was 32?snow 22 mph winds. Silt sock was observed above and below the under access road culvert pipe. Please see attached pictures. Fluid was observed in the secondary containment surrounding the production tanks, but was frozen solid. Bubbling was observed surrounding the Brookfield 1 well head. The OGI supervisor was notified of these findings. A portion of the pad surface appears to be revegetated and stabilized. Plantings were observed along the upper edge of the fill slope. These appear to be grafted apple tree varieties. No violations were cited during this inspection. Please contact me with any questions.

The weather was sunny and 28 degrees. Small amounts of snow covered portions of the pad surface.

There is currently one producing well onsite (1H) with two production tanks. The pad appears stabilized and no erosional issues were observed. Fluids on the pad surface and within containments were frozen.

No issues or violations noted today. I departed the site at 1230 hours.
