On site for a new spud inspection. Met with Brett Wambold of Selman & Associates. United Drilling was on site to drill and run the conductor pipe in the 2H, 3H, 5H, 6H, 9H and 10H wells. The 2H was at TD of 108'. There was an auger rig on the 10H at a depth of 45'. Brett said all the conductors will be set at 108'. There are three existing wells on site, the 1H, 7H and 8H that are temporarily plugged. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2828401 Primary Facility 2019-01-17 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Joel Franks. H&P 259 was on site and drilling the curve of the well when I arrived on site. They were drilling with DOBM. The cement drilled easy when they drilled out the intermediate shoe track. The fit test passed (see photo number 3) and a CBL was run. They then spotted cement and squeezed at the shoe track. I asked that a copy of the CBL be sent to the Department. There are three existing wells on site that are temporarily plugged and six new wells to be drilled. The proposed TD for the well is 12,890'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2835614 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted I previously performed an inspection (inspection number 2790535) in response to a complaint (number 337613) called in, consisting of mud being pulled onto the road from the B. Mead pad. The weather at the time of inspection was in the upper 30s and the skies were overcast with breaks of sun.

The pad was undergoing a drilling operation at the time of inspection, so truck traffic was constantly heavy. There was ongoing sweeping occurring during the inspection, although the roadside was lightly muddy, not just from truck traffic but also from snowmelt. There was a coating of gravel on the access road. It appeared the operator was addressing the issue of debris carryout at the time of inspection.

There were no violations.
