On site for a new spud inspection. Met with company man David Dewberry. United Drilling was on site to drill and run the conductor pipe for the 1H, 2H,3H, 5H, 7H, 8H and 10H wells. There was one auger rig and two air rigs running when I arrived on site. There was an air rig on the 1H at a depth of 7.5'. The 2H was drilled to a depth of 105' and the auger rig was going in to clean out to bottom and run the conductor pipe. The 3H was completed and the conductor pipe was in the hole at a depth of 105'. The 5H was at a depth of 8'. The 7H was at a depth of 6.5'. The second air rig was rigging up on the 8H which was at a depth of 8.5'. The 10H was drilled to a depth of 105'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2750989 Primary Facility 2018-07-09 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as part of an investigation into a nearby water supply complaint in which the water well was impacted by a heavy sediment load on 7/3/18. As Photo #2 shows, new wells have recently been spud on this pad. A review of the most recent Department Drilling Inspection, Inspection Record No. 2748434 found that conductor pipe was installed on 7/3/18 for 7 wells, the 1H, 2H, 3H, 5H, 7H, 8H, and the 10H. The conductors were all drilled to a depth of approximately 105'. During the 7/3/18 inspection, the inspector noted that 3 Rigs were being operated, one auger rig and two air rigs. No E&S issues were identified. No violations noted.
2760527 Primary Facility 2018-08-06 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man William Smith. PD 181 was on site to drill and set the surface and intermediate casings. There are two existing wells on site that are temporarily plugged and there are seven new wells on site to be drilled. They are on the third of the seven wells. They were drilling the hole for the intermediate casing on the 10H well. They are drilling with WBM. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2830124 Primary Facility 2019-01-22 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Andy Rodriguez. Keane was on site to frac the 1H, 2H, 3H, 5H, 7H, 8H and 10H wells. They were stimulating stage 10 out of 50 stages on the 10H and running a plug and perf in the 2H when I arrived on site. The 1H was completed with 51 stages. There were 20 stages completed out of 58 on the 2H. There were 49 stages completed out of 50 on the 3H. There were 49 stages completed out of 51 on the 5H. There were 21 stages completed out of 23 on the 7H. The 8H wasn't started but will have 30 stages. The water was being trucked to the site. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2850444 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted Today's inspection was conducted as a routine inspection. Present during the inspection was Cabot Site Supervisor Steve Gillman. Stimulation activities have been completed and site is currently being demobilized. Production equipment installation is also occurring. Overall the site appears to have good stabilization. No E&S issues identified. No violations noted.
